- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as applicable and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Solicitation number is 1105588, and is being issued as a Request for Quotation. The solicitation documentation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58, dated May 18, 2012. This acquisition is not set aside for small businesses. The associated North American Industry Classification System Code is 811219, Other Electronic and Precision Equipment Maintenance and Repair with a small business size standard of $19.0 million. The purchase order awarded as a result of this RFQ shall be firm fixed price. Background The National Center for Toxicological Research (NCTR)/Office of Regulatory Affairs (ORA) Nanotechnology Core Facility in Jefferson, Arkansas houses a JEOL Model JSM-6610LV Scanning Electron Microscope (SEM). This SEM core equipment supports the nanotechnology needs of the NCTR and the Arkansas Regional Laboratory (ARL), Collectively referred to as the Jefferson Laboratories Complex of the Food and Drug Administration (FDA) centers located away from the Jefferson Laboratory Campus and studies conducted under the National Toxicology Program. The FDA requires annual maintenance for the existing JEOL microscope system (JSM-6610LV, Serial Number MP13710059) (further details below) for a base period of one year to begin August 24, 2012 and end August 23, 2013, plus four 1-year options years, (each period to be separately priced). The period of performance shall not exceed 5 years. The microscope system consists of the following items: a. JSM-6610LV Scanning Electron Microscope b. MP-53020TMP Turbo Molecular Pump EVAC Unit 372587 c. MP-64090BEIW Backscatter Electron Unit 373249 d. MP-61130SNS Stage Navigation System 366838 e. MP-64080LSED Low Vac Secondary Electron Unit 372501 f. MP-50010 PCD Probe Current Unit 360465 g. Transmitted Electron Unit TED-59 Annual maintenance shall include as a minimum: a minimum one preventive maintenance visit performed by an OEM trained and certified technician that includes repair, calibration and free replacement parts, components, subassemblies, etc. (except consumables); software and firmware upgrades; unlimited telephone support from 8 am to 5 pm Eastern Time, Monday through Friday; priority response for unscheduled field and factory service within 24 hours; absolute adherence to and conformity with OEM specifications, manuals, bulletins, etc. in performing all preventive maintenance, services, and repairs. The Provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this acquisition. An offeror shall complete only paragraph (b) if applicable, of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition with the following addenda: This solicitation incorporates one or more FAR and HHSAR Clauses and provisions by reference, with the same force and effect as if they were given in full text. The full text may be accessed electronically at this/these address(es): http://www.acquisition.gov and http://dhhs.gov/asfr/ogapa/acquisition/acquisitionpolicies.html. 352.202-1 Definitions (JAN 2006) 52.217-5 Evaluation of Options (JUL 1990) 52.217-9 Option to Extend the Term of the Contract (MAR 2000) (a) "within one(1) day of maintenance expiration;" (c) "The total duration of this contract, including the exercise of any options under this clause, shall not exceed five years." 52.222-48 Exemption from Application of the Service Contract Act to Contracts for Maintenance, Calibration, or Repair of Certain Equipment Certification (FEB 2009) Service Records and Reports The Contractor shall, commensurate with the completion of each service call (inclusive of warranty service), provide the end-user of the equipment with a copy of a field service report/ticket identifying the equipment name, manufacturer, model number and serial number of the equipment being serviced/repaired and detailing the reason for the service call, a detailed description of the work performed, the test instruments or other equipment used to affect the repair or otherwise perform the service, the name(s) and contact information of the technician who performed the repair/service, and for information purposes, the on-site hours expended and part/components replaced. In addition, the contractor shall provide monthly reports to the FDA COR and Contract Specialist, not later than the 5th work day following the end of each month, summarizing all maintenance and repair activities (including warranty work) for the previous month; negative reports are required. Preventive maintenance and repair visits shall not be scheduled during Federal Holidays or Federal Closures as determined by Executive Orders or WWW.OPM.GOV. Federal holidays are as follows: New Year's Day Martin Luther King Jr.'s Day Washington's Birthday Memorial Day Independence Day Labor Day Columbus Day Veteran's Day Thanksgiving Day Christmas Day Offerors are afforded the opportunity to inspect the system by contacting the Contract Specialist/Contracting Officer identified herein to schedule an appointment. Failure to inspect the system will not relieve the successful offeror from fully meeting the requirements of the resulting contract at the price offered. Evaluation: Proposals shall be comprised of three components: Price, technical approach and past performance. Technical approach and past performance when combined are more important than price. Offers shall describe their proposed maintenance plan and the approach for its implementation. The plan shall also demonstrate proof of certification from the OEM that they are qualified to perform maintenance and repairs on stated equipment (or like equipment) as well as have access to parts, service manuals and all other tools required to perform this maintenance requirement. Proposals from offerors not possessing the OEM certification will not be evaluated. Past Performance: Offerors must furnish information for a minimum of three maintenance contracts for like equipment including customer contact point and telephone number. Offerors who fail to complete the required representations and certifications, reject the terms and conditions of this solicitation or that do not possess an OEM certification for performing maintenance will be excluded from consideration. All responsible sources may submit an offer, which if timely received, shall be considered. The offer must reference solicitation number 1105588. The RFQ is due in person, by postal mail, fax or email to the point of contact listed below on or before close of business on July 18, 2012, Jefferson, AR 72079.
