- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ) W912L5-12-T-0010 The NAICS is 621498, SIC 8021. Small business size standard is $10,000,000.00. This acquisition is Set-aside for Small Business. Joint Force Headquarters, SRP, First Regiment Road, Wilmington, Delaware 19808-2191 will be the main location for examinations and treatment and examinations. New Castle, Kent and Sussex County locations of the Delaware Army National Guard will be considered. The Delaware Army National Guard (NGDE) requirement is to place a Dental Blanket Purchase Agreement for a three year period. This BPA is a non-binding agreement to either party. A thirty day notice is required to terminate the BPA by either party. The BPA is to provide examinations and restorative dental care for Class 3 soldiers upgrading to a Class 2 or 1. Soldiers are on alert orders and/or eligible through Army Selected Reserve Dental Readiness System (ASDRS). Service will include Periodic Health Assessments (PHA), to include dental screening, examinations, digital bitewing radiographs, digital panoramic radiographs, testing, dental treatments and restorative services. Clinical evaluation by a DELAWARE licensed dentist of the oral cavity using visual, clinical exam supported by bitewings taken by certified dental technicians. All services (except extensive Class 3b and 3c Soldiers that require in-clinic treatment and or dental reconstruction) must be initiated on site at NGDE Armories, training sites, and Facilities across the state. The contractor will provide fully equipped Mobile Pods at designated locations that are essential to treat estimated 100 - 150 soldiers per day during a drill weekend. Dental services are to be available Monday through Sunday, operating hour's 0700 hrs to 2000 hours, except Federal holidays or if a government facility is closed due to emergencies. Overtime is not authorized. In accordance with the Performance Statement of Work, the contractor shall provide, all management, labor, material, equipment, certifications and supplies necessary to provide Denclass data entry of all radiographs and dental records, identification, treatment and are qualified personnel with current Delaware State Dental Licensure. Medical Operational Data System (MODS) (www.mods.army.mil) AND MED-CHART (https://medchart.ngb.army.mil) modules will be performed by awarded vendor and NOT sub-contracted data entry by second or third-party entities... The Contractor Personnel are to perform the dental services and/or OSHA, HIPAA, ADA, and Privacy Act Compliant. The Contractor is responsible to obtain a Security Facility clearance and CAC enabled to input reports into the Denclass and Medpros program upon service completion. All services are to be documented, reported and mailed within a designated time period to the point of contact assigned. The offeror must demonstrate in the offer the capability to meet all contract requirements through past performance at similar events, either as the prime or major subcontractor, or by identification of key personnel that have past performance experience. The Government intends to evaluate quotes and make award without discussions with offerors (except clarifications as described in FAR 15.306(a)). Do not assume you will have the opportunity to clarify, discuss or revise your offer. Offerors may, at the discretion of the Government, be asked to provide more information and clarification regarding their offer/quote. Request for such information does not constitute discussions. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. CLIN 0001: Exam, Caries Risk Assessment, Periodontal Assessment, Tobacco Risk Assessment, Oral Cancer Screening, Radiographs. Clin 0002: Treatment/restorative Services, Endodontic Services, Periodontal Services, Prosthodontic Services, Prosthodontic Services, Oral Surgery Services, Orthodontic Services. Contract Line Item Numbers (CLINs): *Note-The Government may award each CLIN separately per FAR 52.212-1. Each CLIN stands on its own. Show alternative pricing if pricing will be more advantageous if both CLINS 0001 and 0002 are awarded. FAR 52.212-1, FAR 52.212-3. Individual calls will be assigned for each soldier; not to exceed $2500 per soldier, completed service. Paperless invoicing will be required monthly not to exceed $150,000.00. The Performance Work Statement attached provides strict guidance. A Contracting Officer Representative (COR) will be assigned to review the BPA and complete a Quality Assurance Report monthly.
