- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. The purpose of this Sources Sought Announcement is for market research to gain knowledge of potential qualified sources interested and capable of performing the work and obtain their size classification (Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a) HUBZone, Women Owned Small Business (WO), Small Disadvantaged Business (SB), etc.) relative to the North American Classification System (NAICS) Code 621610 with a standard size of $13.5M. The Department of Veterans Affairs Medical Center (VAMC), Beckley, West Virginia is seeking potential sources from health care providers capable of providing In-Home Health Aide Services. Services will include providing all In-Home Health Aide Services, as authorized by VAMC, to VAMC patients in their homes. Care shall be provided regardless of race, color, religion, sex, or national origin. The VAMC staff will obtain services from an agency other than the contracted agency, when, by their professional assessment, the other agency would be the better resource to meet the needs of the patient and/or family. In Home Health Aide services shall include but not limited to: (a). Activities of Daily Living (ADL) which includes; Bathing, dressing, toileting, transferring, feeding, and continence. (b). Instrumental Activities of Daily Living (IADL): Shopping for the patient, meal preparation, light housekeeping, medication administration reminder, using telephone. (c)Homemaker Services: Services that is necessary for the maintenance of the patient in the home. Services may include housekeeping, laundry, meal preparation, grocery shopping for the patient, assuring patient safety, and other services that are consistent with the homemaker's level of responsibility and patient plan of care. (d) Home Health Aide Services: Personal care services provided in the home. Services may include assistance with ADL's, aid in ambulation or transfers, active and passive exercises, assistance with medical equipment, and routine health monitoring. (e) Services may also include specific household tasks to maintain a safe environment in areas of the home used by the patient (e.g. changing the bed and doing laundry essential to the cleanliness of the patient.) These services are provided by persons who have successfully completed appropriate training and education consistent with state law and regulations. The VA Community Health Nurse Coordinator (CHNC) will determine the need for H/HHA services based upon the directive and medical center policy governing the program. Consults for these services will be initially ordered by the patient's primary care provider. Once the Veteran has been screened and services are deemed appropriate by the CHNC based on governing directives and policies, a referral will be made to the H/HHA agency. Hours will not exceed 15 hours/weekly. Hours will not exceed 3 hours a day. The home care agency will be responsible for scheduling the H/HHA's in such a way the Veteran will receive all of his/her authorized hours each week. SERVICING AREA The following counties are included in the Beckley VA Medical Center's servicing area and include areas both in West Virginia and Virginia. West Virginia: Raleigh, Fayette, Mcdowell, Greenbrier, Pocahontas, Nicholas, Summers, Monroe, Mercer, Wyoming Virginia: Tazwell, Bath, Alleghany Potential Sources who are interested and capable of providing the above services must demonstrate their experience and knowledge of the required services and the capability to provide such services. Potential sources should identify business name, point of contact, business size, and any applicable business socio-economic category (i.e. small business, SDVOSB, VOSB, SB, etc), service capability statement, information for recent and relevant projects similar in nature, DUNS number, address and any additional information felt to be pertinent. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought Announcement, a solicitation announcement may be published at www.fbo.gov website. Responses to this Sources Sought Announcement are not considered adequate responses to any solicitation announcement that may result from this notice. All interested offerors will have to respond to the solicitation announcement in addition to responding to this Sources Sought Announcement. Written responses to this notice shall be electronically mailed in PDF or word format to Barbara.hawk@va.gov or a hard copy mailed to: US Department of Veterans Affairs, Hampton, VAMC, VISN 6 NCO, 100 Emancipation Drive, Bldg, 27, Attn: Ms. Barbara Hawk, Hampton VA 23667. Telephone responses will not be accepted. Written responses must be received no later than 1:00PM (EST), Tuesday, July 3, 2012. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued, it will be announced separately from the responses to this announcement.
