- Rio Blanco County Area Wage Determination Lost Park Map Performance Work Statement (PWS) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. AG-82X9-S-12-0135. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-58. The North American Industry Classification System Code (NAICS) for this acquisition is 561730 with a size standard of $7.0 million. The proposed contract is 100 percent set aside for small business. CONTRACT: Weed spraying Services for the Blanco Ranger District, White River National Forest Meeker, Colorado. Line Item 0001 Base Period of Performance QTY: NTE 75 Acres, UNIT PRICE: _____________, EXTENDED PRICE: ________________. Period of Performance: 01 August 2012 through 30 September 2012. Line Item 0002 OPTION PERIOD 001 QTY: NTE 75 Acres, UNIT PRICE: _____________, EXTENDED PRICE: ________________. Period of Performance: 01 August 2013 through 30 September 2013. Line Item 0003 OPTION PERIOD 0002 QTY: NTE 75 Acres, UNIT PRICE: _____________, EXTENDED PRICE: ________________. Period of Performance: 01 August 2014 through 30 September 2014. Offerors shall include prices and descriptive literature containing sufficient technical documentation to establish a bona fide capability to meet this requirement. Only written responses will be considered. Offerors shall submit two professional references for this type of service with the following information: Name/POC Address of POC Dates of performance (must be within the last 24 months) Aggregate Value of contract Total acres serviced Contractor must also specify the proposed position(s) from the area wage determination The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation lowest priced, technically acceptable with a neutral or positive past performance evaluation. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If additional information is required to support price reasonableness, each offeror may be required to provide a hard copy of their price break-down. Non-conformance with this requirement may result in the offerors quote being determined unacceptable.
