This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for commercial items, and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract, Small Business Set-Aside Requirement. Only qualified offerors may submit bids. Solicitation VA-249-12-Q-0651 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation as a written solicitation will not be issued. NAICS Code is 517210 and size standard is 1500 employees. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in CCR @ www.ccr.gov and be capable of providing all travel, labor, service coverage, materials, tools, documentation and parts required for the support and repair of the equipment, software, and accessories for the Alphanumeric pagers in accordance with all terms, conditions, provisions, schedules and specifications listed in the attached statement of work. This service is required for the VA Tennessee Valley Healthcare System (TVHS), 1310 24th Avenue South, Nashville, Tennessee and 3400 Lebanon Pike, Murfreesboro, Tennessee, a two-campus facility within VISN 9, as well as Clarksville and Chattanooga, Tennessee Out Patient Clinics (OPC), in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on all itemized items and total base year including all option years pricing as provided below.
