- Hydrogen Peroxide SOW Standard Form-18 Invitation Memo The Phoenix Indian Medical Center (PIMC) has a requirement for Preventive Maintenance Services to be provided in Phoenix, Arizona. Prospective Offerors are invited to submit proposals in reference to Request for Proposal No. 12-247-SOL-00062. The Government anticipates an award of a Firm Fixed Price contract. The Government intends to award one contract as a result of this solicitation. The Contractor shall provide Preventive Maintenance Services (PM) and repairs on the Large and Small Hydrogen Peroxide Sterilization Equipment in the PIMC Central Supply Room to meet the manufactory specification requirements, National Fire Protection Association (NFPA) 101, 99, and the Standards for Health Care Facilities and Environment of Care (EC.02.04.01, EC.02.04.03) Joint Commission Standards. The Preventive Maintenance Services shall be performed for the following equipment: Large Hydrogen Peroxide Sterilizer: Model: 100S, S/N: 10101984204 Small Hydrogen Peroxide Sterilizer: Model: NX, S/N: 33101626 The initial contract will be one (1) year, with two (2) option years to extend. The solicitation will be available on or about June 22, 2012 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. The Offerors are required to submit RFQ by email to: bradley.platero@ihs.gov. The RFP due date is close of business 12:00 pm MT on June 28, 2012. Late proposals will not be accepted. All contractors doing business with the Federal Government shall be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov.
