- Request for Proposal Document NOTE: This Acquisition is set-aside for Small Businesses only. The NAICS Code applicalbe is 236220. The Business Size Standard for that NAICS Code is $33.5 Million average annual revenue. For consideration, the Offeror must have average annual revenue at or below that level. 1. PROJECT SUMMARY The Contractor shall perform a variety of work at healthcare facilities throughout the Phoenix Area Indian Health Service (IHS) area of responsibility. The work will include but not be limited to performing building assessments, construction inspection, building code interpretation, minor engineering design, construction estimating or other related work associated with facilities engineering design, construction, operation and maintenance. The services shall be provided at IHS health care facilities located at various locations in Arizona, California, Nevada and Utah. Work may be required to be performed at a variety of installations from small remote health stations to a full-service major medical center. Work may also be required at Government housing. It is anticipated a majority of the work under this contract will be at remote locations. 2. DESCRIPTION OF SERVICES The Contractor employee(s) shall perform the tasks required at IHS facilities, both local to Phoenix, AZ and at remote locations. Such Contractor Employee(s) will be provided workspace at the IHS Phoenix Area Office and work under the direction of the Chief, Operations Branch, Division of Facilities Engineering, Office of Environmental Health and Engineering. The Chief, Operations Branch, Division of Facilities Engineering will provide Contractor Employee(s) a prioritized listing of tasks that shall be performed, with a schedule for each location. The Chief of the Operations Branch will also define project objectives, general methods, priorities, and deadlines for each major assignment. Contractor Employee(s) shall discuss unusual task problems with the Chief, Operations Branch, before taking action to resolve them. The Contractor shall provide all basic tools necessary to perform the duties. The Government will provide any required specialized tools and equipment. The Contractor Employee(s) shall follow established written guidelines and procedures (design standards, standard specifications, programmatic policies and regulations). The Contractor will perform field and office duties associated with government-owned facilities including evaluation of existing deficiencies, design of minor repair projects, construction inspection and project management of contract work, and oversight of in-house construction projects. Typical types of facilities-related components where the Contractor may be required to perform functions are: (a) Building Envelops: i. Roofs ii. Walls iii.Doors iv.Windows v. Foundations (b)Grounds: i. Streets, roads, curb & gutter ii. Parking Lots iii.Landscaping iv.Irrigation systems (c)Building Interior Structures: i. Walls, floors, ceilings (d)Building Systems: i. Electrical 1.Lighting 2.Power distribution 3.Stand-by or Emergency Power Generation ii. Plumbing 1.Drain, waste and vent 2.Domestic water systems iii.Mechanical iv.Fire Protection The Contractor may be required to perform skilled technical work such as infrared imaging or backflow prevention testing. Certifications for skilled technical work, when required, may be obtained by the Contractor at the Government's expense. 3. PLACE OF PERFORMANCE & WORK HOURS The base location for the Contractor employee(s) provided for these services will be at the Phoenix Area Office, Office of Environmental Health and Engineering Division of Facilities Engineering, 40 N. Central Avenue, Suite 720, Phoenix, Arizona. The Government will provide office space and allow the Contractor use of Government hardware, software and other tools necessary to perform required tasks. Contractor Employee(s) shall work in and from this facility for remote requirements. Normal work hours are 8am to 5pm Monday through Friday, excluding federal holidays. The contractor will be compensated for only the hours Contractor Employee(s) work. 4.REQUIRED QUALIFICATIONS (a)The on-site contractor staff shall possess the following minimum qualifications: i. A minimum of three years full-time experience in all phases of construction or construction management. Experience shall include but not be limited to: concrete, framing, HVAC, electrical, plumbing, roofing and interior finishes. ii. Demonstrated knowledge and understanding of the following codes: National Electric Code (NEC); Uniform Building Code (UBC); Life Safety Code (LSC); Uniform Mechanical Code (UMC); and the Uniform Plumbing Code (UPC). iii.Physically able to climb ladders (up to 20 feet), enter crawl spaces, and carry equipment up to 40 pounds in total weight. iv.Demonstrated knowledge in reading and interpreting detailed construction plans and specifications. (b)All Contractor Employees proposed for this work must possess at least one certification or registration related to construction, inspection or operation and maintenance of facilities in the following specialties. i. Electrical Systems, infrared imaging; ii. Water systems, backflow prevention device inspection and testing iii. Fire Hydrant inspection and testing iv. Fire Pump and/or motor inspection and service v. Portable Fire Extinguishers and/or Kitchen Hood Extinguishers vi. Medical Gas Alarm and Outlet Testing vii.Home Inspections viii.Biological Containment Cabinets Testing and Service ix. Uninterruptable Power Supply Testing and Service x.
