Description
Evaluation Criteria Statement of Work and Deliverables Schedule This is a Request for Quotation. A quote is being requested and a written solicitation will not be issued. Requirement number OCSQ-393-2012-0113 is issued as a Request for Quotation. This requirement is being set aside for small business concerns. The Centers for Medicare and Medicaid Services (CMS) has a requirement for a contractor to provide consulting support services to the Resident Assessment Instrument/Minimum Data Set (RAI/MDS) activities through the services of a national expert who is knowledgeable regarding federal long-term care surveyor regulations and guidance as related to the implementation of the MDS 3.0. Detail information is provided in the attached Statement of Work. The level of effort is 936 hours. The period of performance will be from the date of award through 12 months, approximately July 9, 2012 through July 8, 2013, plus an additional Option Year. This will be a firm fixed price procurement using simplified acquisition procedures. Your response may be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 3:00pm on July 2, 2012 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, C2-17-06, Baltimore, MD 21244. Responses shall be no more than 10 pages and should include the following: 1) Contractors capabilities overview; 2) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables, base year, plus the additional Option Year. Your response will be evaluated on a best value basis using the following evaluation criteria: 1) Technical and Management Approach - the likelihood of effectively meeting requirements; 2) Past performance - the relevance and quality of prior performance; and 3) Price - the amount, realism and consistency of the evaluated price. All contractors are hereby advised that contractors shall be registered in the Central Contractor Registry (CCR). Please register with the CCR at the following website: http://www.ccr.gov. Questions and inquiries must be submitted in writing no later than 3:00pm on June 25, 2012 to: Lucille.Lee@cms.hhs.gov and Phillip.Harrell@cms.hhs.gov. Responses and clarifications will be provided in an amendment to the solicitation by June 29, 2012.
