- Description
- The Marion Veterans Affairs Medical Center Cardiology Department wishes to contract for preventative maintenance and repair for two GE Ultrasound Vivid 7 Machines. The intended procurement will be classified under North American Industrial Classification (NAICS) code 423450 with a size standard code of 100 employees. This will be a Service Disabled Veterans Small Business (SDVOSB) set aside. The contractor shall furnish all labor, transportation, and parts necessary to provide preventive maintenance and repair of the two GE Ultrasound Vivid 7 Machines with serial numbers V74958, 7018V7L located in the Marion, IL VA Medical Center Cardiology Department. Each respondent must have an established business, with an office and full time staff to include a "fully qualified" field service engineer (FSE) and a "fully qualified" FSE who will serve as backup. "Fully qualified" is based upon training and on experience in the field. For Equipment training, the FSE(s) must show that they have successfully completed a formalized training program on the equipment. For field experience, the FSE(s) must demonstrate that they have a minimum of two years experience with respect to scheduled and unscheduled preventive and remedial maintenance of this equipment. The FSE(s) shall be authorized by the contractor to perform the maintenance services. All work shall be performed by "fully qualified" competent FSE(s). The contractor shall provide written assurance to the competency of their personnel and a list of credentials of approved FSEs for each make and model that the statement of work covers. The Contracting Officer (CO) may authenticate the training requirements and request training certificates or credentials from the contractor at any time for any FSE personnel who are servicing or installing the equipment discussed in the statement of work. The CO and/or Contracting Officer's Technical Representative (COTR) specifically reserves the right to reject any of the contractor's personnel and refuse them permission to work on the VA equipment. If subcontractors are used, they must be approved in advance by the CO. The contractor shall be required to provide the same qualifications for all subcontractor FSEs as identified in the statement of work Any contractors selected for award will be expected to comply fully with these requirements. The suggested firm-fixed price contract will be a base year and three (3) option years. The base year of contract will commence on July 1, 2012 thru June 30, 2013. The contract award basis is scheduled to be a single or multiple awards based on offeror(s) responses and their location to the VAMC Facility. The contractor will be required to perform all maintenance and support services referred to them from the Department of Veterans Affairs, Marion, IL serviced by this contract. Interested vendors should review FAR Clause 52-212-1 Instruction to Offerors for submittal instruction and Far Clause 52.212-2 Evaluation Commercial Items for Evaluation factors for the solicitation. Only vendors registered in the Central Contractor Registration (CCR) and the Veterans Enterprize VETBIZ data base will be considered for contract award.
