Description
This solicitation amendment -03 corrects the error in block 13 of the SF-1442 -- the SF-1442 shows the time for receipt of proposals as 2:00pm in block 10 and 5:00pm in block 13 -- the correct time is 2:00pm. Proposals are due at 2:00pm on July 2, 2012. The Forest Service contemplates issuing solicitation no. AG-0116-S-12-0068 for the Middle Ridge Road Surfacing and Reconditioning project on or about May 29, 2012, with offers due on or about July 2, 2012. The estimated magnitude of the project, with all options, is between $500,000 and $1 Million. This project involves the following work: (1) Producing, hauling and placing unclassified borrow to cap existing oversize rock and for use as required to meet a maximum road grade of 18% on NFSR 50054 (milepost 0.0 – milepost 1.0) and NFSR 50060 (milepost 0.0 – milepost 1.9). Option 2 includes furnishing, hauling and placing a 2 ft. lift of unclassified borrow on the 150 diameter circular log transfer site at the end of NFSR 6250. (2) Producing, hauling, placing and compacting crushed aggregate on NFSR 50054 from milepost 0.0 to milepost 1.0 and on NFSR 50060 from milepost 0.0 to milepost 1.9. Two options extend aggregate to milepost 3.5 on NFSR 50060 and add crushed aggregate to NFSR 6250. (3) Adding HDPE culvert on NFSR 50054 and NFSR 50060. (4) Reconditioning the roadbed of NFSR 50054, milepost 0.0 to milepost 1.0, and NFSR 50060, milepost 0.0 to milepost 3.5. Option 2 includes reconditioning of roadbed on NFSR 6250. (5) End hauling of material removed during ditch cleaning (reconditioning) on the last 600 ft. of NFSR 6250 is included in Option 2. End haul material is to be disposed of in the quarry located at milepost 0.2 on NFSR 6250. (6) Developing existing quarries for shot rock production and for crushed aggregate production. The quarry at milepost 0.6 on NFSR 6255 is designated for both borrow and aggregate production and the quarry at milepost 3.1 on NFSR 50060 is designated for shot rock production. (7) Seeding and fertilizing soil disturbed during reconditioning work on NFSR 50054 and NFSR 50060. Option 2 extends seeding and fertilizing to include NFSR 6250. (8) Producing and stockpiling crushed aggregate in the quarry located at milepost 0.6 on NFSR 6255 is included as Option 3. (9) Mobilization of Equipment to perform the work. Metal and other manufactured products excavated will become the property of the contractor and disposed of by removing from Government-administered lands and properly disposing of. Included are metal culverts removed and items encountered during ditch cleaning. The intent of this contract is to provide for the complete construction of the project described in the contract. Unless otherwise provided, the Contractor shall furnish all labor, materials, equipment, tools, transportation, and supplies and perform all work required to complete the project in accordance with drawings, specifications, and provisions of the contract. A total small business set-aside is contemplated. Offerors may obtain a copy of the solicitation from this FBO website. The Forest Service will not mail paper copies. This information will be used to formalize a set-aside decision, now contemplated to be total small business.
