- DEPARTMENT OF VETERAN AFFAIRS SOUTHEAST LOUISIANA VETERANS HEALTH CARE SYSTEM P.O.BOX 61011 New Orleans LA 70161-1011 Notice Date: 06/07/2012 Set Aside: N/A Notice Type:Special Notice IAW: F.A.R 6.303-2 This is not a request for proposal or a request for quote. There is no solicitation documentation available Under F.A.R Part 6.303-2. This is a notice of intent. The Southeast Louisiana Veterans Health Care System intends to negotiate a sole source firm fixed price contract with SONSITE, INC. 21919 30TH Dr. SE Bothell WA 98021-3904. The items to be purchases are: P07680 3 C60X/5-2MHz transducer biopsy compatible C60X/5-2MHz multi-frequency, broadband, 16mm curved array transducer for general-purpose, abdominal, obstetrics, gynecology, and nerve exams. Supports 2-D with SONOMB multi-beam technology and auto gain capabilities, SONOHD imaging technology, tissue harmonic imaging, M-mode, color power Doppler, velocity color flow Doppler and pulse wave Doppler -Biopsy guide available. Meter drop test durability. Maximum Depth 30 cm, the purchase equipment must be compatible with the existing system. The Government intends to procure these items under the authority of U.S.C. 2304 (c) (1) as implemented by FAR 6.302-1. A determination by the Government not to open the requirement to competition based on responses to this notice is solely within the discretion of the Government. Oral communications are not acceptable in response to this notice. Interested parties may identify their interest and capability to respond to this requirement no later than June 15, 2012 4:00 PM CST offers submitted past the appointed time shall not be considered. All information submitted should support the offers capability to provide services regards to the Sole Source Justification and Approvals at no cost to the Government. The Sole Source Justification and Approval is available upon request. Responses must be in writing, and must provide clear and concise documentation, indicating an offerors ability to meet the services required. Responses will be emailed to Gary.Baker3@va.gov verbal responses are not acceptable and will not be considered. Emails are not to be in the excess of 5MB. Prospective offerors shall complete electronic annual representations and certification at http://orca.bpn.gov, shall be registered in the Central Contractor Registration (CCR) database at http://www.ccr.gov prior to award of the contract.
