Description
QUICK-FIX STATEMENT OF WORK Project Title: Building 47812 Remodel Project Number: AF Form 332 Project Site: Building 47812, JBER-Richardson 673D POC: Mike Marberg 384-0216 SCOPE OF WORK: The Contractor shall provide all labor, equipment, materials, supplies, handling and storage, transportation, supervision and quality control to perform all work, including work of an incidental nature, TO REMODEL OFFICE SPACE. The work requires the Contractor to provide and install all materials required to complete the work. This project includes, but is not limited to the following work: REMOVAL AND DISPOSAL •1. Remove and dispose of file cabinets in room E and area F on attached drawing. •2. Remove and dispose of the work surfaces, wall tracks, bookshelves, and stand-alone desk in area F. •3. Remove and dispose of wood shelving on wall in room B. (Large shelving to be removed, small shelving stays) •4. Remove and dispose the base cabinets, countertop, and wall cabinets from area K. •5. Remove and dispose table in a K •6. Remove and dispose of refrigerator in kitchen area. NEW CONSTRUCTION •1. Construct a floor-to-ceiling wall approximately 4-ft long in kitchen area. New wall will be sheetrock on both sides, primed and painted. •2. Install carpet in areas F & K and also in rooms A, D, E. Carpet shall be a minimum of 28oz. level loop commercial grade glue down carpeting.. •3. Install Vinyl Composition Tile in rooms B, G, H, I and also in area L. •4. Install scraper floor matting in area C (Foyer). •5. Install 4" vinyl cove base throughout the facility. •6. Install transitions between carpeting and tile areas. •7. Remove screws/nails from walls, patch all holes on walls throughout the facility to a smooth finish, prime and paint with one two coat s of paint. •8. Paint all ceilings, radiator covers, trim, and existing doors. •9. Colors for carpet, tile, cove, and paint to be chosen by Mr. Bob Duellman. •10. In room B, install a 4' x 6' x ¾" piece of plywood on the left wall as you enter the room. •11. In rooms A, D, and E, install sound dampening tiles (floor to ceiling) as shown in the drawing. •12. Install vinyl, horizontal mini-blinds on all windows. •13. Replace the faucet in the kitchen sink. •14. If feasible, replace existing thermostats with digital/programmable thermostats. •15. Repair/replace hand cranks for windows 3, 4, 5, and 7 as shown on the drawing. •16. Remove existing countertop in kitchen and replace with Corian style counter and backsplash. •17. Repair hole in ceiling in room A. •18. Repair the metal support leg under the sink in the men's restroom. ELECTRICAL / COMMUNICATIONS / IDS ALARM SYSTEM •1. In room B (LAN room), install a dedicated 120-volt, 20-amp NEMA 520 Duplex receptacle for a LAN system. Also, install one dedicated 120-volt, 20-amp standard receptacle for a telephone system. •2. Install a LAN drop and a telephone outlet in rooms A, D, E. In Area F, install three (3) LAN drops and two (2) telephone outlets. In Area K, install four (4) LAN drops and three (3) telephone outlets. Total of 10 LAN drops and 8 telephone outlets. All areas are marked on attached map. •3. All LAN drops will terminate in the room B (LAN room). CAT 6 Data cable will be used. •4. All telephone drops will terminate in Area M (mechanical room) and CAT 3 Telephone cable will be pulled in. •5. In room A, convert duplex receptacle to a quad receptacle •6. In area K, convert one duplex receptacle to a quad receptacle. Also install one duplex receptacle on south wall. •7. Clean all existing lighting fixture covers and relamp as necessary. •8. Install a low-voltage door bell with chime at Door number 1. •9. Install an AIPHONE (or similar) to control visitor access. Provide one master station in Area K and a sub-master station in room A. All necessary cabling and electrical wiring should be installed to provide a complete and functioning system. •10. Install an electric strike on the door so both the master station and sub-master station is able to release the door when a visitor arrives. •11. All electrical and communication boxes to be recessed in wall if possible. If unable to do so, surface mount is acceptable with wiremold raceway or similar. •12. Submittals for all equipment must be provided prior to purchase and installation for approval by Mr. Bob Duellman. LOCKS •1. Install a basic 5-button cipher lock at door number 1. This lock should be outdoor rated. This lock should have the capability to retract the latch to allow visitors to enter the foyer during regular duty hours. •2. Install a KABA E-PLEX 5800 PACS (or similar) on door 2 as shown on attached drawing. •3. Install a KABA E-PLEX 5800 PACS (or similar) on door for room B. •4. Submittals must be provided prior to purchase and installation for approval by Mr. Bob Duellman. BUILDING EXTERIOR •1. Scrape, caulk, and paint the exterior of the facility. Paint scheme to match JBER paint plan/scheme. SITE VISIT: It is required that the Contractor visit the work site to verify location and thoroughly familiarize himself with existing conditions prior to submission of bid. APPLICABLE CODES: All work in this contract shall be done in accordance with National Fire Protection Association (NFPA), Uniform Building Code (UBC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), National Electrical Safety Code (ANSI C2), OSHA-Occupational Safety and Health Act, EM 385-1-1, US Army Corps of Engineers Safety and Health Requirements Manual, ANSI B 20.1 1992, 40 CFR EPA, Environmental Protection Agency, and all local safety, fire and security regulations. This job requires that all personnel have appropriate, applicable and current Certificate of Fitness cards from Alaska Department of Labor. All codes listed here shall be the latest version and most recent or up-to-date publications in print. CONTRACTING AUTHORITY: Only the Contracting Officer is authorized to make changes or additions to the contract. The COR and Government Inspector are authorized to act for the Contracting Officer only to the extent that such authority has been delegated to them in writing by the Contracting Officer. COORDINATION OF WORK: Once the contractors' proposals have been received and reviewed, Contracting will notify the contractor awarded the contract. NOTICE TO ALL INTERESTED PARTIES This requirement will be awarded under the JBER Quick Fix program and will be awarded to one of four prequalified Blanket Purchase Agreement holding contractors and is being posted here solely to satisify the legal requirement that all requirements estimated to be over $25,000.00 in value be advertised to the public.
