- QUICK-FIX STATEMENT OF WORK Project Title: Bldg 13272 Replace Carpet with Tile Project Number: 26832 Project Site: Bldg 13272, JBER Elmendorf, Alaska 673D CES POC: Mike Marberg 384-0216 1. Contractor is to furnish all labor, materials, and equipment to accomplish the following work: Contractor shall remove the existing carpet and associated materials in the entry way and main break area (Rm. 108) and the installation of new Johnsonite Rubber Tile in the break area to match hallway and install a walk off mat in the entry way of building 13272. Repair the surrounding area on all effected walls. This project includes but is not limited to the following tasks: 2. GENERAL SPECIFICATIONS: a. All work will be accomplished in accordance with all applicable building, electrical, and fire codes in effect at the time of award of contract. b. During the job, the contractor is responsible for compliance with all OSHA/AFOSH Standards. c. The contractor's job site is to remain clean at all times. Debris will be collected and disposed of daily. d. During the performance of this project the contractor is responsible for repairing any damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government, repair any damage caused by construction operations, and take all precautions necessary to safely protect the building and its occupants during the construction period. e. Use of premises is limited to the indicated work area. Do not disturb adjacent areas beyond the work site. Only the Government right to work may limit the Contractor's use of the premises. f. The Government will occupy the site and exiting building during the entire construction period. Coordinate with the Government during construction operations to minimize conflicts and facilitate Government usage. g. Keep driveways and entrances serving the premises clear and available to the Government's employee customers and emergency vehicles at all time. Do not use these areas for parking or storage of materials. 3. SPECIFIC REQUIREMENTS: a. Remove old carpet in break room and material in entry and dispose of off base. b. Prep floor for Johnsonite rubber tile and walk-off mat. c. Install new Johnsonite rubber tile and walk-off mat in entry way. d. Color: New rubber flooring will match the existing rubber color scheme and style. e. Remove and replace the existing trim and molding with similar type of trim and molding. f. Contractor will dispose of all unused flooring and debris from the building. Note: All parts will meet safety standards and mee the government's specifications listed above. SITE VISIT: It is required that the Contractor visit the work site to verify location and thoroughly familiarize himself with existing conditions prior to submission of bid. APPLICABLE CODES: All work in this contract shall be done in accordance with National Fire Protection Association (NFPA), Uniform Building Code (UBC), Uniform Mechanical Code (UMC), Uniform Plumbing Code (UPC), National Electrical Safety Code (ANSI C2), OSHA-Occupational Safety and Health Act, EM 385-1-1, US Army Corps of Engineers Safety and Health Requirements Manual, ANSI B 20.1 1992, 40 CFR EPA, Environmental Protection Agency, and all local safety, fire and security regulations. This job requires that all personnel have appropriate, applicable and current Certificate of Fitness cards from Alaska Department of Labor. All codes listed here shall be the latest version and most recent or up-to-date publications in print. CONTRACTING AUTHORITY: Only the Contracting Officer is authorized to make changes or additions to the contract. The COR and Government Inspector are authorized to act for the Contracting Officer only to the extent that such authority has been delegated to them in writing by the Contracting Officer. COORDINATION OF WORK: Once the contractors' proposals have been received and reviewed, Contracting will notify the contractor awarded the contract. NOTICE TO INTERESTED PARTIES This requirement will be awarded under the JBER Quick Fix Program and will be awarded to one of four prequalified Blanket Purchase Agreement (BPA) holding contractors and is being posted here solely to satisfy the legal requirement that all requirements estimated to be over $10,000.00 in value be advertised to the public.
