Description

Statement of Work 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation number RFQ12-PHX-02 is being issued as a Request for Quote (RFQ). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This Request for Quote is being issued under full and open competition. The North American Industry Classification System (NAICS) code is 811219 - Other Electronic and Precision Equipment Repair and Maintenance, and the applicable size standard is $19,000,000.00 in annual revenue. 5. This requirement consists of six (5) line items: 0001 - Preventive Maintenance of Toshiba Nemio U/S 0002 - Option Year 1 Preventive Maintenance of Toshiba Nemio U/S 0003 - Option Year 2 Preventive Maintenance of Toshiba Nemio U/S 0004 - Option Year 3 Preventive Maintenance of Toshiba Nemio U/S 0005 - Option Year 4 Preventive Maintenance of Toshiba Nemio U/S 6. The Statement of Work is attached to this notification. The requirement is for the preventive maintenance and service of the Toshiba Nemio ultrasound machine at the San Carlos PHS Indian Hospital in accordance with the attached Statement of Work. 7. The location of contract performance is 223 Seneca Lane, San Carlos, Arizona 85550, and the point of delivery is FOB Destination to the same address. The anticipated Period of Performance is July 1, 2012 through June 30, 2017 if all options are exercised. 8. INSTRUCTIONS TO OFFERORS: 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation. 9. PROVISIONS/CLAUSES: 52.212-2, Evaluation - Commercial Items: applies to this solicitation.
