- (1) N/A (2) 0606 (3) 2012 (4) 77030 (5) R (6) Department of Veterans Affairs Support Services Division (362/24) 6900 Almeda Road Houston TX 77030-4200 (7) Mental Skills Training IDIQ For Chapter 31 Veterans (8) VA362-12-R-0005 (9) 06132012 (10) John A Player (11) N/A (12) N/A (13) N/A (14) N/A (15) N/A (16) 1.PURPOSE THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs, Houston Regional Office is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work require under the proposed contract. The NAICS code is 624310, Vocational Rehabilitation Services 2.DESCRIPTION OF THE REQUIREMENT The contractor shall provide all services, materials, labor, tools, equipment, transportation, facilities, and supervision to provide Mental Skill Training (Peak Performance Training) Services in accordance with this Performance Work Statement. All services shall be performed in accordance with the contract terms and conditions set forth herein. The Program shall consist of the following elements: ·Four (4) Day, Thirty Two (32) Hour Peak Performance Mental Skills Workshop Content: -Introduction to Peak Performance -Goal Setting -Adaptive Thinking -Stress-Energy Management -Attention Control -Imagery Active Learning -Exercises and activities to understand and apply learned concepts -Biofeedback to demonstrate mind-body connection and self-regulation Materials -Mental Skills Workbook -Exercises and Handouts -Advanced Readings Software and Hardware -EmWave Coherence Trainer for mind-body training -Mindshaper Suite for attention and concentration training -ThinkFast software to measure and train reaction time, perceptual reflex, cognitive reflex, working memory speed, and subliminal perception ·Peak Performance Mental Skills Mastery Training and Coaching (25 to 30 Hours) -Conducted in one-on-one hourly session over the course of the TRACK year with professional performance psychologist and peak performance coach and trainer -Application of learnings from the workshop -Use of biofeedback and neurofeedback to measure and train mental, emotional, and physiological control during performance in high stress situations -Develop self-regulation and self-control skill and applying them in real-world scenarios -Safe and controlled learning environment with the use of Alpha chairs and high tech audio-video equipment 3.FREQUENCY The contractor shall provide services to cohorts not larger than 13 Veterans. The contractor shall schedule two (2) cohorts per year. It is anticipated that the resultant contract will be for a one-year base period plus four (4) option periods. 4.POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE a)Company names, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. b)Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. c)Submit Capability Statements by June 13, 2012 to the Contracting Officer, John A.
