Weather Emergency Operations Center (MWEOC) FEMA RFP HSFE50-12-R-0036 Pricing Spreadsheet List of Provisions and Clauses Applicable to this RFP This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is being issued as Request for Proposal (RFP) No. HSFE50-12-R-0036 for bagged halite (rock salt) and bulk sodium chloride (road salt). The RFP clauses and provisions are those in effect thru Federal Acquisition Circular 2005-58, effective 05/18/2012. This acquisition is being solicited as a total service-disabled veteran-owned small business set aside. The NAICS codes applicable to this requirement are 325998 and/or 424690; small business size standards associated with these NAICS codes are 500 and/or 100 employees, respectively. Single or multiple awards for bagged halite and bulk sodium chloride may result from this solicitation via the issuance of a firm-fixed price simplified acquisition purchase order(s). All deliveries shall be made to DHS, Federal Emergency Management Agency, Mount Weather Emergency Operations Center (MWEOC), 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006. The following line items are required: Item No. 0001, 22 pallets of Halite (rock salt) packaged in 50 pound bags (one truckload of approximately 880 bags) to be delivered upon FEMA's request between September 1, 2012 and March 31, 2013 (please provide unit price per pallet or bag); Item No. 0002, 22 pallets of Halite (rock salt) packaged in 50 pound bags (one truckload of approximately 880 bags) to be delivered upon FEMA's request between September 1, 2012 and March 31, 2013 (please provide unit price per pallet or bag); Item No. 0003, estimated shipping charges per truckload for delivery of Halite (rock salt) to 19844 Blue Ridge Mountain Road, Mt. Weather, VA 20135-2006 (this line item will be an estimated not-to-exceed amount); Item No. 0004, 700 tons of Sodium Chloride (bulk road salt) to delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck between September 1, 2012 and August 31, 2013 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0005, 700 tons of Sodium Chloride (bulk road salt) to delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck between September 1, 2013 and August 31, 2014 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton); Item No. 0006, 700 tons of Sodium Chloride (bulk road salt) to delivered upon FEMA's request in quantities ranging from 45 to 75 tons each via 10 wheel dump truck between September 1, 2014 and August 31, 2015 (this line item will be an estimated not-to-exceed amount; please provide unit price per ton). FOB Destination pricing is requested for line items 0004, 0005 and 0006. Reference the attachment titled "FEMA RFP HSFE50-12-R-0036 Pricing Spreadsheet" for submitting price proposals. Please include all applicable Government discounts in your offer. Source selection will be based on lowest cost, technically acceptable determination. In order to be considered technically acceptable, the contractor must provide evidence that it is a service-disabled veteran-owned small business and is registered in CCR; demonstrate the ability to deliver the required product on a "will call" basis (upon FEMA's request); and, demonstrate the ability to deliver each product in the quantities and units (pallets, bags, tons) requested. FAR Provisions 52.212-1, 52.212-2, 52.212-3, 52.212-4 and 52.212-5 apply to this acquisition. Offerors are to include a completed copy of FAR provision 52.212-3 "Offeror Representations and Certifications--Commercial Items" with their response or provide this information via the ORCA website, http://orca.bpn.gov. The successful vendor must be registered at www.ccr.gov. A list of provisions and clauses applicable to this RFP is provided as an attachment to this combined synopsis/solicitation. Reference the attachment titled "List of Provisions and Clauses Applicable to this RFP". Contractor and Sub-Contractor personnel must comply with MWEOC security requirements. Contractors are required to become familiar with CFR 44, Part 15 and ensure any sub-contractors are also familiar with these rules and regulations before entering the MWEOC facility. Hand deliveries and/or faxes of questions will not be accepted without prior approval. Additionally, the subject line of your email shall, at a minimum, contain the RFP number. PROPOSAL SUBMISSION INSTRUCTIONS - The Contractor shall submit an electronic proposal via email to Phyllis.Woodward@fema.dhs.gov no later than 12:00 PM EST on June 20, 2012. The subject line of emails pertaining to this RFP shall clearly reference RFP # HSFE50-12-R-0036. Total size of email shall not exceed 8MB due to DHS email restrictions. Hand deliveries and/or faxes of proposals will not be accepted without prior approval. Contractor proposals shall be valid for 30 calendar days after the due date for responses to this RFP. A written notice of award or acceptance of proposal, emailed or otherwise furnished to the successful offeror shall result in a binding contract without further action by either party. Amendments to this RFP will be published in the same manner as the initial combined synopsis/solicitation.
