Amendment #1 Note: Please see attachments to view this combined synopsis/solicitation in its entirety. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FDA-SOL-1103893 is being issued as a request for quotation (RFQ). The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. This requirement is being solicited as a Total Small Business Set-aside using Simplified Acquisition Procedures per FAR Subpart 13. A firm-fixed price purchase order is anticipated. The associated North American Industrial Classification System (NAICS) code for this procurement is 562211, with a small business size standard of $12.5 mil. The U. S. Food & Drug Administration (FDA), Office of Acquisitions and Grant Services (OAGS) is seeking to purchase hazardous waste removal and disposal services at the FDA facility located in Winchester, MA. All interested companies shall provide a quotation for the following: The contractor shall provide all necessary labor, permits, disposal arrangements, hauling, and equipment, as necessary, to properly remove and dispose of radioactive hazardous waste. All work must be accomplished in accordance with applicable federal, state, and local laws, rules, and regulations. The contractor shall be licensed by the appropriate accrediting body and must comply with RCRA Subtitle B - Hazardous Waste Regulations (40CFRParts 260-279), Nuclear Regulatory Commission (NRC) Regulations (10CFRParts 0-199 (specifically Part 20)), and the Occupational Safety and Health Standards (29CFR1910).
