The Department Of Veterans Affairs (VA), Veterans Integrated Service Network (VISN 2), which consists of Stratton VA Medical Center, 113 Holland Avenue, Albany, NY 12208; VA Western New York (WNY) Healthcare System, Buffalo Campus, Buffalo VA Medical Center, 3495 Bailey Avenue, Buffalo, NY 14215, VA Western New York Healthcare System, Batavia Campus, 222 Richmond Avenue, Batavia, NY 14020, Syracuse VA Medical Center, 800 Irving Avenue, Syracuse, NY 13210, Bath VA Medical Center, 76 Veterans Avenue, Bath, NY 14810, Canandaigua VA Medical Center, 400 Fort Hill Avenue, Canandaigua, NY 14424, Rome Outpatient Clinic, Griffiss Business Park, 125 Brookley Road, Building #510, Rome, NY 13441 and Rochester Outpatient Clinic, 465 Westfall Road, Rochester, NY 14620 (from here forward known as VISN 2), intends to award a Firm Fixed Price Requirements Service Contract to a qualified firm with the capability and capacity for classifying, segregating, bulking, packaging, labeling, storing, transporting, treating and disposing of hazardous waste from various VISN 2 hospital and CBOC locations. Services shall be in accordance with the Statement of Work of the contract. Period of Performance will be a base period of August 1, 2012 (or as soon as possible after award) through July 31, 2013, with the possibility of four (4) one year options (August 1st thru July 31st) for renewal. The resulting contract will be subject to the Service Contract Act. The applicable NAICS code is 561112 and the size standard is $12.5 million. Telephonic requests for the solicitation will not be accepted. A Live Oral Face to Face Presentation to be performed at the Syracuse VA Medical Center, 1304 Buckley Road, Syracuse, NY 13212-4311. Time and Date to be determined after review of submitted bids. Solicitation is expected to be issued May 31, 2012 and will be available electronically at https://www.fbo.gov. All responses are due 2:00 PM (EST), June 29, 2012. Offers from all qualified, capable and responsible sources will be considered. Vendors offering a response to this notice should ensure registration in the Central Contractor Registration database at http://www.bpn.gov/ as well as listed (certified) the Vendor Information Pages (VIP) @ www.vetbiz.gov IAW VAAR 804.1102 prior to submission of offers for consideration. Offerors are required to submit a complete copy of the provision 52.212-3 - Offeror Representations and Certifications - Commercial Items, direct responses to 52.212-2 Evaluation - Commercial Items with their offer.
