- EVALUATION FACTORS STATEMENT OF WORK This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Food and Drug Administration intends to award a competitive purchase order for Attending Veterinary Services for the White Oak, Maryland Animal Program. Request for Quote (RFQ) # FDA-SOL-12-1104293. The offeror shall submit a technical proposal that covers the work described in the statement of work. The technical proposal shall not exceed 10 pages exclusive of resumes and past performance. Resumes of the primary and backup AV are required. The offeror shall submit a business proposal that includes hourly rates (fully loaded) for all labor categories. The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-30. This requirement is for full and open competition under NAICS 541940 "Veterinary Services" ($6.5 million size standard). In order to appropriately perform this task, the veterinarian must be licensed and have obtained the status Diplomate, ACLAM, at least 10 years of experience. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition.. Offerors should include a copy of the provision FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with their quote. FAR 52.212-4, Contract Terms and conditions - Commercial Items applies to this acquisition. FAR 52.212-5, Contract Terms and conditions Required to Implement Statues or Executive Orders - Commercial Items applies to this acquisition.
