Description
Request for Quote Performance Work Statement Wage Determination Revision 11 "This proposed contract is being considered as a set-aside under a small business set-aside program. The North American Industry Classification Systems (NAICS) Code proposed for the requirement is 611620. The size standard for NAICS 611620 is $7M. The government is interested in all small businesses including (8(a), Historically Underutilized Business Zone, Service Disabled Veteran Owned Small Businesses, or, if applicable based on NAICS code, Women Owned Small Businesses that are interested in performing this requirement. The government requests interested parties submit a brief description of their company's business size (i.e. annual revenues and employee size) business status (i.e., 8(a), Historically Underutilized Business Zone, or Service Disabled Veteran Owned Small Businesses, Women Owned Small Business or small business) anticipated teaming arrangements, and a description of similar services offered to the Government and to commercial customers. Any responses involving teaming agreements should delineate between the work that will be accomplished by the prime and the work accomplished by the teaming partners. The Government will use this information in determining its small business set-aside decision." This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) F4D3F12110AA01-1 Submit written offers - Oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) (www.ccr.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-58 and DFARS Change Notice 20120518. Description of Service - AEROBICS INSTRUCTOR - 1. Performance period will be from 18 Jul 2012 through 17 Jul 2013, Basic Year, with (3) one-year options from 18 Jul 2013- 17 Jul 2016 plus one six-month extension of services through 17 Jan 2017. 2. Acceptance and inspection of all services will be at Vandenberg AFB, CA. 3. Offers shall conform to the CLIN structure as established in this synopsis/solicitation. 4. Offers shall be valid for 30 days after close of solicitation. 5. Attached Performance Work Statement (PWS) and latest Wage Determination 2005-2063, Revision 11, dated 13 June 2011 are applicable. 6. All questions relating to this solicitation must be received by 10:00 am PDT on 14 June 2012, in order to ensure adequate time to answer them. 7. CLIN 1001 Aerobics Instructor, Basic Year, (QTY) 12, (Unit) Months, Period of Performance: 18 July 2013 - 17 July 2014. CLIN 2001 Aerobics Instructor, Basic Year, (QTY) 12, (Unit) Months, Period of Performance: 18 July 2014 - 17 July 2015. CLIN 3001 Aerobics Instructor, Basic Year, (QTY) 12, (Unit) Months, Period of Performance: 18 July 2015 - 17 July 2016.
