Description
This is a request for quote for repair of Grissom Air Force Base VORTAC site. Vendor will provide all materials, supervision, labor and equipment to perform miscellaneous repairs according to the attached Scope of Work. Site is located in Miami County, Indiana. The FAA will not pay for any information received or costs incurred in preparing the response to the request for quote. Therefore any cost associated with the request for quote submission is solely at the interested vendor's expense. The competition will be based on lowest price. This competition is open only for small businesses, or service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SBA for participation in the SBA's 8(a) Program. Contractors must be registered in the Central Contract Registry (https://www.bpn.gov/ccr/default.aspx) prior to submitting a response. Interested firms are required to submit the attached Business Declaration form and a copy of their SBA 8(a) certification letter, if applicable. In order to make this determination the FAA requires the following from interested vendors: 1. Pricing, utilizing the attached bid sheet. 2. Capability Statement - Vendors must document they possess the following minimum qualifications and experience: - Minimum of 2 prior projects replacing VORTAC counterpoises, including installation of a membrane roof. - Manufacturer-certified for the installation of EPDM roofing. - Minimum of 2 years of experience installing EPDM roofs. - Proven record of staffing and executing a VORTAC counterpoise reproofing project in 18 days or less. - Able to work on United States Government installations. Attached are the Installation Specifications and Drawings, Terms and Conditions, Bid sheet, Wage Determination and Business Declaration Form. Please respond by reviewing these items, and submitting a completed, signed pricing sheet, capability statement and a filled-out and signed copy of the Declaration form (please submit electronically, email to address below). All responses to this request for quote must be received by Thursday, June 14, 2012, 2:00 CST (Oklahoma Time).
