Description

The Department of Veterans Affairs, Iowa City VA Medical Center, Iowa City, IA intends to negotiate a SOLE SOURCE procurement under the authority of FAR 6.302.1, FAR 6.302.5, with Hocoma, Inc. 77 Accord Park Drive, Norwell, MA 02061. Interested parties may identify their experience in providing these services to the Contracting Officer via e-mail at melissa.hutchison2@va.gov or fax 319-887-4957. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-59, effective May 15, 2012. The associated NAICS code is 334510 and small business size standard is 500 employees. The acquisition is set-aside and is being negotiated as a SOLE SOURCE procurement. The requirement for this acquisition includes the following: ITEM DESCRIPTION OF QTY UNIT AMOUNT NO. SUPPLIES/SVCS 1 Disassemble, pack, ship and 2 EA reassemble the following Lokomat system at the assigned sites, noted below: LS033 - VA Medical Center Washington, DC to VA Medical Center Iowa City, IA pricing includes two [2] Hocoma Certified Technicians for two [2] full days at Standard daily rates of 1,400 per day, per technician 2 Trucking fees will be billed at 1 EA actual cost incurred. Dedicated transportation from point to point 3 Standard travel fees 2 EA per technician, per installation 4 Maintenance Program: 1 EA Lokomat serial numbers: LS0033 service validity: one year from date of Clinical Training of each system yearly required maintenance hotel and travel expenses of the Hocoma service technician additional cost: spare parts; service visits on demand [repairs in between yearly required maintenance or extra services]. travel costs {travel, hotel, etc.] are billed at standard Service on Demand rates. 5 User training on demand: 1 EA basic training [Lokomat User] the participants in this training have on experience with the Lokomat System. after passing the theoretical examination they become Lokomat User. Maxinum 6 participants are allowed to a training. Duration of the training is 2 days 6 Additionally, travel costs 1 EA [travel expenses & hotel accommodation] will be charged for the Hocoma training expert for both training options based on a standard fee per half day. 7 Advanced training [certified 1 EA Lokomat User] participants have already accomplished Lokomat initial training. after having successfully completed the advanced training, each participant will be certified as a CERTIFIED LOKOMAT USER and, additionally, receives a personal copy of the user manual. Each certified Lokomat User is allowed to provide Lokomat User trainings at his or her center himself. Maximum 6 participants are allowed to a training. Duration of the training is [1] day 8 Additional travel costs 1 EA [travel expenses & hotel accommodation] will be charged for the Hocoma training expert for both training options based on a standard fee per half day.
