Description

It is the intent of the Syracuse VA Medical Center, 800 Irving Ave., Syracuse, NY 13210, to negotiate on a sole source basis (IAW FAR 6 and 12) with Biometrics LTD, PO Box 340, Ladysmith, VA 22501, 804-448-2520. This equipment is provided on a commercial basis, but the only source of this specific equipment in this specific configuration is Biometrics LTD. This procurement is being conducted under the authority of Far 6.302-1. Only one responsible source and no other equipment or training will satisfy agency requirements; therefore, full and open competition is determined inappropriate for this acquisition. This notice is for information only and may represent the only official notice of such solicitation/synopsis, however, the Government will consider all proposals received on or before due date identified below. A determination by the Government not to compete this proposed procurement based on the responses to this notice is solely within the discretion of the Government. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. This requirement is to provide the equipment and freight. Part number - EP12 E-Link Exercise Package Typically used in Stroke Units and Neuro Rehabilitation oUnilateral and Bilateral Single-axis Evaluation and Exercise of Weight Distribution, Stability, and Sway oHand, Upper & Lower Extremity Exercise oBack & Neck Exercise oDocumentation and Progress Reporting Includes: oX4 InterX Unit oE4000 Upper Limb Exerciser oM600 - Myo-EX (whole body EMG for exercise)& AngleX (whole body active exercise against gravity) oH500 - Grip & pinch assessment and exercise oDFP2 (2 ForcePlates, 1 BF8 BaseFrame, 2 Connecting leads used when connecting 1 or 2 ForcePlates to the X4) oSoftware - Exercise software for all included components, grip & pinch evaluation, ForcePlate single axis assessment The EP12 includes all the components to make one complete E-LINK evaluation and exercise station using your own computer, printer, and table. NAICS Code is 334510, Size Standard, 500. Submissions received in response to this notice shall be evaluated based on the technical ability and demonstrated past experience to meet the requirement.
