Description
Gerald R. Ford Job Corps is soliciting cost proposals for a qualified professionl to provide subcontracted dental services to students (ages 16-24). The licensed dentist will provide preliminary dental exams to each weekly class of new student arrivals; plus planned dental care for the student body. The dental clinic is located within the student wellness clinic and is staffed with a subcontract dental assistant/hygienist. Services are to be provided for a period of 12 hours per week, 48 weeks per year on a fixed priced service basis. Services are to include: Providing a mandated oral exam, and the development of an oral health and wellness plan; providing a written diagnosis and treatment plan as part of the plan for each student; establishing treatment according to priority classification and within program constraints; providing basic dental care, as defined by the Job Corps Procedures and Requirements Handbook; providing oral health education; establishing an appropriate referral system; providing or aranging for 24-hour emergency coverage; participating with other center components to coordinate and integrate an oral health program; maintaining accuracy and confidentiality of all required oral health and wellness records; providing support to the students in acquiring good oral health skills, knowledge and attitudes; promoting opportunities for students to practice the skills which will make them employable; developing and complying with Job Corps infection control policies and procedures; and other duties as defined by the Center Director, Technical Assistance Guide, and compliance agencies and requirements. Proposal must contain a copy of the current State of Michigan license, a qualifications brief (sesume'), DEA number, proof of malpractice insurance (1,000,000 per occurence/3,000,000 aggregate), and a bid for the monthly/hourly rate. Job Corps is a national training and employment program administered by the Department of Labor for disadvantaged youth. Awarded subcontractor will be awarded for the current Option Year 2 (beginning June 1, 2012 - May 31, 2013), and Option Year 3 (June 1, 2013 - May 31, 2014). These and further Awards are contingent on continued government funding.
