Description

RFP document SF-33 The Nashville Area, Indian Health Service, is proposing to contract non-emergency medical transportation services in the Mashpee, MA area under a blanket purchase agreement (BPA). Enclosed is an RFP numbered 285-12-RFP-0020 with a scope of work outlining the services desired. Please read the entire proposal document. If you would like to submit on the proposed work please construct an offer for the compensation related to your services as defined in the following proposal and complete all of the documentation included in the package. The standard form (SF), SF-33 must be completed along with any other signature blocks for consideration. If you are not a "CCR" registered contractor you may follow the steps in section 52.204-7 and fill out the application to become registered. Please response no later than June 4, 2012.
