Description
PRE-SOLICITATION SYNOPSIS FOR CRYOTHERAPY EQUIPMENT SERVICES 11 MAY 2012 INTRODUCTION: This is a pre-solicitation synopsis stating that the United States Air Force is contemplating issuing a solicitation for Cryotherapy equipment services to be performed at the 88th Medical Group (hereafter known as the MTF), Wright-Patterson Air Force Base, Ohio. This pre-solicitation synopsis is intended for informational purposes only. The NAICS code for this action is 811219 (Other Electronic and Precision Equipment Repair and Maintenance) size standard $13.5 Million. 1.2 DESCRIPTION OF SERVICES: The contractor shall perform the tasks outlined in the following paragraphs to fulfill the requirements: 1.2.1 Contractor shall provide cryosurgical equipment (CRYOgrid, Integrated Biplane Ultrasound) and all related cryosurgical disposable supplies (CRYOprobes, TEMprobes, and Urethral Warmer Catheter Kit) required to perform cryosurgical procedures, as scheduled by Hospital, and shall provide a technician during scheduled procedures. 1.2.2 The contractor shall perform the Services in accordance with the applicable standards of professional practice, and comply in all material respects with applicable laws, medical staff bylaws, and policies of Wright-Patterson Air Force Base, including those relating to patient confidentiality and record retention. 1.2.3 Provide coverage on a scheduled basis, with normal operating hours occurring Monday thru Friday, 0700-1600, excluding Federal holidays; the contractor shall respond on a non scheduled basis, within 60 minutes at the discretion of attending surgeon. 1.2.4 Provide coverage for non-scheduled cases, and shall have a response time within sixty minutes, after initial notification. Additional time may be authorized at the discretion of the attending surgeon. 1.2.5 Provide and maintain adequate liability insurance coverage consistent with the risks associated with the performance of all services required. Refer to Federal Acquisition Regulation (FAR) Clause 52.237-7, "INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE", in Section I of this contract. A certificate of insurance showing coverage limits shall be provided to the Contracting Officer upon award of this contract. 1.2.6 Provide a basic price rate which includes all supplies and equipment (contractor provides all consumable supplies necessary for services including, but not limited, to sub dermal needle electrodes, various other electrodes and tape.) 1.2.7 Transport, maintain, repair and calibrate contractor owned equipment used in performing the required services. The contractor shall provide documentation of maintenance repair and calibration which shall be provided upon request of the Government. Prior to use, any equipment brought into the facility must be cleared through the Medical Group's Medical Equipment Repair Center (MERC) prior to use on patients. After normal duty hours the MERC on-call person shall be contacted for clearance of equipment. 1.2.8 Contractor shall provide documentation for the list prices of the items and services included in each procedure within 48 hours upon request. MTF RESPONSIBILITIES 2.1.1 Provide the contractor with patient information: Name, Phone, DOB, Diagnosis, medical history, physician, procedure being performed, estimated length of service required, procedure date and estimated start time. 2.1.2 Provide the operating room, staff and non cryosurgical supplies and the required argon and helium gases. 2.1.3 Provide the contractor with a list of Government personnel authorized to place calls against the contract for service. 2.1.4 Retain full responsibility for billing patients of any other third parties for Services performed by contract personnel and paid for by the MTF. 2.1.5 Provide the contractor with the information needed for obtaining Base access. 2.1.6 The Government reserves the right to seek other providers in the event any contractor is not available to provide the services on the date required or if the contractor fails to meet standards in this BPA.
