Description
The Departmental Appeals Board requires maintenance on two Video Teleconference (VTC's) Suites and the associated peripherals. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6 in conjunction with FAR part 13, as supplemented with additional information included in this notice. This announcement constitutes the solicitation and quotes are being requested. The solicitation number is 12-233-SOL-00297 and it is issued as request for quotation (RFQ). This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35. This RFQ is a total small business set-aside. Statement of Work This document specifies customer requirements and responsibilities as it pertains to the maintenance of two Video Teleconference (VTC) suites and associated peripherals for the Health and Human Services Departmental Appeals Board (DAB). 1. Overview The Departmental Appeals Board (DAB) use its VTC suites in direct support of its Medicare and Medicaid Appeals hearings. Integrated to our VTCs is the For the Record (FTR) hardware and software solution to provide the DAB and complainants with the hearing transcripts in a digital format. The FTR deck provide us with the ability to record local room audio, videoconference (audio only) or audio conference meetings via a digital recording device. Additionally, the system will support playback of the previously recorded audio during our VTC supported hearings. This document defines the standard professional services required to ensure accurate and consistent services are delivered. This statement of work covers work to be performed in the following location: Departmental Appeals Board (DAB) 800 N. Scope of Work The contractor shall provide the necessary personnel, materials, services, parts, and otherwise do all tasks necessary to perform the work and services called for under this Scope of Work. Purchase orders issued under the contract will be for support to the Departmental Appeals Board (DAB), a division of the Department of Health and Human Services (DHHS) at their Washington DC location. The contractor shall furnish as may be required and ordered by the Government, SERVICES which include adequate maintenance for the two VTC suites and components specified below. Contractor Requirements Contractor will provide the labor and materials required to support and maintain the aforementioned conference room system and its peripherals. Upon receipt of the signed HHS SOW and purchase order approval the contractor will provide: • Assignment of a Project Manager (PM) to ensure current operations effectiveness, system integration, and support to any problem or issue with system • 5 x 12 Technical Support with 2 hour response • Qualified and certified personnel • Next Business Day On-Site Maintenance • Next Business Day Shipment for codec and MCU parts replacement • Two Business Day Shipment for parts on other major hardware and peripherals • Software Updates per manufacturer licensing agreements • Remote Diagnostic Support • PM will contact customer prior to dispatch any part or personnel to customer site. • Develop project plan to highlight milestones and project duration 4. Customer Responsibilities • Verify room availability for scheduled maintenance date and reserve or reschedule install if necessary • Provide power where required • Provide network connectivity • Provide and test ISDN network connectivity prior to contractor arrival on-site • Provide ISDN network information to contractor point of contact • Designate a Single Point of Contact (SPOC) to resolve implementation issues and coordinate the scheduling of site repairs and maintenance. 5. Deliverables The equipment and/or material or supplies required will be contractor supplied. The contractor shall provide a variety of services as detailed on request and service calls. The contractor shall complete all work according to this work statement. The contractor shall be responsible for adherence to all federal, local, and site-specific regulations. The period of performance for this purchase order shall be one full year after date of award. Funding for this requirement is limited to $5,878.46. All items are to be delivered FOB destination no later then 30 days after award to 330 Independence Ave, SW, Cohen Building, Washington, DC, 20201. The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2 is not applicable. The Government will award a purchase order resulting from this synopsis/solicitation to the lowest priced quote that meets all of the technical requirements. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (End of clause) 352.231-70 SALARY RATE LIMITATION (MAR 2012) (a) Pursuant to the current and applicable prior HHS appropriations acts, the Contractor shall not use contract funds to pay the direct salary of an individual at a rate in excess of the Federal Executive Schedule Level II in effect on the date an expense is incurred. (b) For purposes of the salary rate limitation, the terms "direct salary," "salary", and "institutional base salary", have the same meaning and are collectively referred to as "direct salary", in this clause. An individual's direct salary is the annual compensation that the Contractor pays for an individual's direct effort (costs) under the contract. Direct salary excludes any income that an individual may be permitted to ear outside of duties to the Contractor. Direct salary also excludes fringe benefits, overhead, and general and administrative expenses (also referred to as indirect costs or facilities and administrative [F&A] costs). Note: The salary rate limitation does not restrict the salary that an organization may pay an individual working under an HHS contract or order; it merely limits the portion of that salary that may be paid with Federal funds. (c) The salary rate limitation also applies to individuals under subcontracts. If this is a multiple-year contract or order, it may be subject to unilateral modification by the Contracting Officer to ensure that an individual is not paid at a rate that exceeds the salary rate limitation provision established in the HHS appropriations act in effect when the expense is incurred regardless of the rate initially used to establish contract or order funding. (End of clause) HHSAR 352.237-73 NON-DISCRIMINATION IN SERVICE DELIVERY (MAR 2012) It is the policy of the Department of Health and Human Services that no person otherwise eligible will be excluded from participation in, denied the benefits of, or subjected to discrimination in the administration of HHS programs and services based on non-merit factors such as race, color, national origin, religion, sex, gender identify, sexual orientation, or disability (physical or mental). By acceptance of this contract, the contractor agrees to comply with this policy in supporting the program and in performing the services called for under this contract. The contractor shall include this clause in all sub-contracts awarded under this contract for supporting or performing the specified program and services. Accordingly, the contractor shall ensure that each of its employees, and any sub-contractor staff, is made aware of, understands, and complies with this policy.
