Description
DESCRIPTION: the VA Nebraska Western Iowa Health Care System intends to negotiate a commercial (FAR Part 12) sole source contract with Diane Kammer, Savage MN, under the authority of FAR 6.302-1. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) FAC 2005-53. Only one responsible source and no other supplies or services will satisfy agency requirements. A justification for other than full and open competition is being prepared in support of this acquisition. This contractor has knowledge and experience with the current Chemotherapy Safety Initiative Program that would not be readily available to another contractor. The following is the Statement of Work we are requesting this contractor to complete for us. VISN 23 Chemotherapy Safety Initiative and Innovations in Consult Management Scope of Work for Oncology Nursing Consultant FY2012 Background Information The Chemotherapy Safety Initiative (CSI) is a VISN 23 quality improvement project that continues to examine a variety of strategies to enhance the safety and effectiveness of oncology care for veterans with cancer. This project has been expanded to include a newly funded program whose aim is to enhance specialty care delivery to veterans via e-consultation or other non-face-to-face means. VISN 23 Oncology Service has been a model for tele-oncology and now has the opportunity to spread e-consultation by utilizing other electronic process, including the use of decision support software. The current Oncology Nurse Consultant, has been under contract and working with the VISN 23 Oncology Chemotherapy program since 2010. During this time significant progress has been made in successfully meeting and exceeding performance thresholds for VISN and VHA Oncology performance measures. The use of an Oncology Nurse Consultant has become a model for the provision of quality oncology nursing care for other VISN's to emulate. Scope of Work/Contractor Responsibilities The Consultant will work with the Chemotherapy Safety Initiative (CSI) Medical Director and medical/nursing oncology personnel providing chemotherapy services at the following VISN23 facilities: Minneapolis VA Healthcare System One Veterans Drive Minneapolis, MN 55417 Fargo VA Healthcare System 2101 Elm Street Fargo, ND 58102 Sioux Falls VA Healthcare System 2501 West 22nd Street Sioux Falls, SD 57117 VA Nebraska Western Iowa Healthcare System 4101 Woolworth Ave Omaha, NE 68105 VA Black Hills Healthcare System 113 Comanche Road Fort Meade, SD 57741 Iowa City VA Healthcare System 601 Highway 6 West Iowa City, IA 52246 VA Central Iowa Healthcare System 3600 30th Street Des Moines, IA 50310 St Cloud VA Healthcare System 4801 Veterans Drive St Cloud, MN 56303 The consultant will provide consultative assistance to nursing personnel, which involves using expert nursing recommendations and clinical expertise not currently available in each oncology nursing area to address global patient care issues. These nursing issues may involve:: "Intra-facility mentoring "Cross-discipline subject matter sharing "Principles of chemotherapy administration and monitoring "Programmatic and material educational resources "Assistance win the development and training of personnel for e-consultation models that are site-specific yet meet the national requirements The consultant in the role of collaborative coach, faculty member and facilitator shall: "Collaborate with the CSI Medical Director and facility nursing personnel to develop lesson plans for local and VISN-wide continuing education on various topics related to oncology nursing such as Principles of Chemotherapy Administration, Management of Patient Problems Associated with Chemotherapy Administration, Latest Trends and New Advancements in Cancer Treatment, Oncology Case Management, Chronic Pain Management, Palliative Care, and other topics as identified through a needs assessment process. Each training program will be approximately 2 hours in length and be delivered virtually using Videoconference and Live Meeting technology. "Collaborate with facility medical and nursing personnel to develop and utilize evidence -based, cost -effective chemotherapy administration policies "Promote latest methods for enhanced patient-centered care via e-consultation. Specific deliverables "Moderate monthly nursing conference calls "Produce documents as requested for policy development "Produce content as requested for website development "Promote the value of and assist in the attainment of certification for currently practicing nurses by providing written information to nurses regarding the oncology certification process, providing resources to assist nurses in studying for the certification examination, being accessible by telephone or email within 2 business days to answer questions from nurses preparing for certification "Collaborate with VA employees in the development and standardization of a nursing case management model for interested VISN 23 sites "Respond to questions from clinical personnel at VISN 23 sites regarding national survey requirements "Assist in the development and spread of e-consultation, the cancer collaborative, and other patient-centered projects as they are identified "Review patterns of practice to determine learning needs "Tabulate appropriate and inappropriate documentation for remediation "Create tables, graphs and other summary documents related to care activities throughout the network, as requested "Contribute to production of chemotherapy order sets in multidisciplinary meetings "Record meeting activities and provide minutes as requested "Produce verbal and written e-consultation implementation status updates for project director as requested "Interview stakeholders, record and report concerns related to e-consultation, as requested "Prepare and present educational overview on virtual care for VISN Cancer Collaborative Services Include: Conference Calls / Data Review Estimate time - 240 hrs Planning / Collaboration Estimate time - 160 hrs Education in-service Estimate time - 400 hrs Electronic Chemotherapy Ordering System Work Group Estimate time - 80 hrs THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. All capabilities statements received by 12:00 P.M. Central Time, May 17, 2012, will be considered by the Government. A determination not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement.
