- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. STATEMENT OF WORK BLANKET PURCHASE AGREEMENTS FOR CATERING SERVICES Ohio Army National Guard REQUESTOR USP&FO for Ohio Ohio National Guard REQUESTOR LOCATION 2811 West Dublin Granville Road Columbus, Ohio 43235 DESCRIPTION THE GOVERNMENT INTENDS TO AWARD A MAXIMUM OF TWO (2) INDIVIDUAL 1 YEAR PLUS 4 OPTION YEARS BPA CONTRACTS FOR CATERING. This action is being solicited as a total small business set-aside. The BPAs for catering services are needed to feed our Soldiers at key times throughout the year. The Government will award a Firm Fixed Price (FFP) contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The following factor shall be used to evaluate offers: Lowest price which meets the standards within the SOW. FAR Clauses which will be in the contract are: 252.232-7003 (WAWF), 52.212-4, 52.212-5, 52.217-9 NOTE: Only qualified offerors registered in CCR (www.ccr.gov) will be considered for this RFQ. Vender must reference their TIN, CAGE CODE and DUNS # in their quote. The submitted quote must take into consideration all of the requirements that are as follows: INSTRUCTIONS, CONDITIONS, AND NOTICES to OFFERORS CONTENT AND FORMAT OF SUBMISSION Quotation submittal and all other required documents must be submitted in accordance with this RFQ. Failure to submit the aforementioned information correctly shall make your offer non-responsive, and it shall not receive consideration for award. Please provide a complete price quote and attach a copy of your CAGE Code, DUNS Number, Tax Identification Number, discount terms, and expiration date to your quote. A quotation submitted in response to this solicitation must be submitted electronically and be comprised of the following Sections: Section 1: Company's Technical Qualifications (All below items must be in relation to the attached Statement of Work) a. Company's Overall Technical Capabilities b. Must supply a 4 week sample menu c. Company MUST be within a 45 mile radius of the place of performance. d. Additional Information: **Contractor shall submit invoices via WAWF. Payment terms are NET 7 days after the submission of the invoice from the contractor and the goods receipt from the Government. SUPPLIES OR SERVICES TO BE PROVIDED This is a Firm Fixed Price (FFP) type Blanket Purchase Agreement that will be awarded via OPEN MARKET as set forth in the Statement of Work included herein. This award shall be made via price and most recent past-performance. BPA Single Call Limit: $50,000 BPA Ceiling: $300,000 PERIOD OF PERFORMANCE Contract starts from date of award until 30 September 2012 with four possible (4) option years thereafter. Period: June 1, 2012 - 30 September, 2012 + 4 possible additional option years a. Base Year: April 1, 2012 - 30 September, 2012 b. Option Year 1 - October 1, 2012 - September 30, 2013 c. Option Year 2 - October 1, 2013 - September 30, 2014 d. Option Year 3 - October 1, 2014 - September 30, 2015 e. Option Year 4 - October 1, 2015 - September 30, 2016 The submitted quotes MUST provide pricing for the base year and pricing of each option year thereafter. QUESTIONS: All questions must be submitted NLT 17 May 2012. RFQ will close and no additional proposals will be accepted after 22 May 2012. SERVICE DESCRIPTION The Ohio National Guard needs a Vendor to provide catering services to feed our soldiers at key times throughout the year. Meals may be served in a designated building on base or in a field environment. Vender must be flexible with locations, dates, times and headcounts. Note: meal count will possibly vary per day depending on how many soldiers are scheduled. The vendor will have a one week notice of the changes. A final headcount will be provided one week before the next class starts. Billing will be adjusted accordingly. DELIVERY REQUIREMENTS/LOCATIONS: On site locations will vary between 2 locations: 1) Rickenbacker Air Force Base (RANGB) 2) Defense Supply Center Columbus (DSCC) Meals may be served in a designated building (TBD) on base or in a field environment. Vender must be flexible with locations, dates, times and headcounts. MENU REQUIREMENTS 1.References: a.AR 30-22, The Army Food Program b.DA PAM 30-22, Operating Procedures for the Army Food Program. c.AR 40-25, Nutrition Standards and Education. d.OHARNG Food Service SOP 2.Project Scope: Provide full food service catering in order to provide daily hot breakfast, boxed lunches and evening meals (Dinner) in support of the Ohio Army National Guard. The daily menu will be in accordance with the above references. The contactor will provide a cycle menu which will include required components of a nutritionally balanced meal. A.Evening Meal minimum requirement: "One meat entr e, one vegetarian entree, appropriate sauce or gravy to accompany entre, one soup either cream or broth based, one starch, one vegetable, a grain item (bread or rolls), fresh fruit, a daily salad, desert, condiments, and beverages, and 1% milk. B.Breakfast minimum requirement: "Eggs (scrambled), one breakfast meat, one hot cereal (grits, oatmeal, farina), assorted grain items (bread-biscuit), one potato choice, condiments, coffee, juice, assorted cold cereal, beverages, and 1% or 2% milk - milk can be a mix of chocolate and white however must have white. C.Lunch requirements: "Boxed Lunches handed out at breakfast. "1 sandwich (4oz meat/2oz cheese with multi-grain or roll, fruit (cup or hard-skinned) bottled water, chips or pretzels, cookies, condiments, plastic ware and napkins. 3. Period of Performance: 01 June - 30 September 2012. Breakfast serving time: 6:00-7:00am, Dinner serving time: 5:00pm-7:00pm. (SUBJECT TO CHANGE) SCOPE OF WORK VENDER'S REQUIREMENTS: A)Include occasional catering out into the field. B)Provide meals 7 days a week. C)Soldiers may be fed in an armory dining facility or possibly in the field. D)Tables and chairs are provided. F)Must provide plates, eating utensils, napkins, cups, and condiments. G)Must serve the meals to the soldiers or drop meals off upon request depending on requirements for that day. H)Must be able to accommodate any soldier dietary requirements i.e: allergies, vegetarian. I) MUST BE FLEXIBLE AND TO BE ABLE TO ADJUST TO THE TRAINING SCHEDULE WITH TIMES, DATES AND HEAD-COUNT. J)If serving: Server will monitor food serving area, maintain a clean food area, maintain foods at safe temperatures, and replenish food and beverage items as necessary. K)Catering services include set-up, breakdown, and clean-up of catering area and dispose of leftovers. L)Constant coordination with the Contracting Officer Representative (COR). M) Upon completion of catering services, invoices are to be submitted to via: Wide Area Workflow (WAWF) per contract instructions. Terms will be NET 7. N)Must submit with their bid a sample of a 1 month menu for the Breakfast, Lunch and Dinner meals. REQUESTOR REQUIREMENTS A)Notify vendor of any changes in meal count or special requirements 1 week in advance. B)Contract Officer Representative (COR) will coordinate specifications and event requirements with vendor C)Will provide tables to support serving line area, dining tables, and chairs. WORK SERVICE GUARANTEE Food & Food service staff be in compliance with health & safety code requirements - MUST SUPLY A COPY OF MOST RECENT COUNTY HEALTH INSPECTION CERTIFICATE/DOCUMENTATION.
