Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. North Atlantic Regional Contracting Office (NARCO) intends to competitively award a commercial Non-Personal Services contract, firm-fixed price, for Pediatric Physical Therapy services located at Keller Army Community Hospital, West Point, NY. The solicitation is restricted 100% Set Aside for Service-Disabled Veteran-Owned Small Business concerns, to be awarded to the lowest priced technically acceptable (LPTA) offeror. NAICS Code: 621340, size standard $7.0M. Keller Army Community Hospital, West Point, NY, has a valid requirement for Pediatric Physical Therapy services. The contract will be awarded for a base period of three months with four consecutive option periods of twelve months each. The total contract potential period is 51 months. Estimated start is on or about 01 July 2012. Specific responsibilities include a complete understanding of the scope of work, and how this requirement will be accomplished; from the implementation of the contract, through daily service. The contractor shall understand the size of the project and will provide full service, at high levels of quality, all aspects of this project, including daily production of administrative support services throughout the life of the contract. The contractor shall provide high quality, accurate and timely reports. Addendum to FAR 52.212-1, Instructions to Offerors, in the solicitation provides submission details, date for receipt of offers and evaluation basis.
