Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and shall not obligate the Government to award a contract. The purpose of this Sources Sought Announcement is for market research to gain knowledge of potential qualified sources interested and capable of performing the work and obtain their size classification (Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, 8(a), HubZone, Women Owned Small Business, Small Disadvantaged Business, etc.) relative to the North American Classification System (NAICS) Code 621999 with a size standard of $10.0 Million. The Department of Veterans Affairs (VA), Asheville, NC, VA Medical Center (Asheville VAMC) is seeking potential sources from health care providers capable of providing Mobile Lithotripsy Services, Extracorporeal Shock Wave Lithotripsy (ESWL) equipment and related technical support laser technicians or RNs at the Asheville VAMC, NC, 2 Friday's per month for an estimated 36 patients per year. The Asheville VAMC estimates that there will be 1 to 3 patients per visit. The contractor shall provide technical and licensed personnel trained and certified in EWSL and operation of the device to Asheville VAMC Monday through Friday, 7:30AM to 4:30PM. All Lithotripsy specialists shall be certified by the American Registry of Radiologic Technologists. All Contractor personnel shall have unrestricted licenses in a US state, territory or Commonwealth of the US or District of Columbia. The Technician or RN shall assist the attending Urologist and other physicians in the operation of the device. The technician or RN is required to operate the device and any related imaging equipment during procedures. The Contractor shall provide a mobile, portable state of the art, digital unit that can be easily moved from place to place into the operating room at the Asheville VAMC. The VA requires at a minimum, a Transportable STORZ Modulith brand or equivalent system. The equipment must meet or exceed the following criteria: 1. Treat stones anywhere in the urinary tract. 2. Have a large focal point to allow for maximal positioning. 3. Have a greater than 80 percent success rate, based on FDA clinical study data. 4. Modular unit to treat patients in the operating room. 5. Have the ability to do endourologic procedures before and after ESWL for kidney stones without moving the patient. 6. Accommodate patients up to 400 pounds. 7. Have the capability to adapting to the existing electrical outlets in the operating rooms. The Contractor shall provide such additional materials and supplies, including electrodes and other consumables that are necessary to conduct ESWL procedures with the device and shall provide maintenance on the device to include service and preventative maintenance to insure the device is functioning at an optimal level. Potential Sources who are interested and capable of providing the above services must demonstrate their experience and knowledge of the required services and the capability to provide such services. Potential sources should identify business name, point of contact, business size and any applicable business socio-economic category (i.e. small business, SDVOSB, VOSB, SB, etc), service capability statement, information for recent and relevant projects similar in nature, DUNS number, address and any additional information felt to be pertinent. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought Announcement, a solicitation announcement may be published at www.fbo.gov website. Responses to this Sources Sought Announcement are not considered adequate responses to any solicitation announcement that may result from this notice. All interested offerors will have to respond to the solicitation announcement in addition to responding to this Sources Sought Announcement. Written responses to this notice shall be electronically mailed in PDF format or word format to sandra.bennett3@va.gov or a hard copy mailed to: US Department of Veterans Affairs, Hampton VAMC, VISN 6 CAS, 100 Emancipation Drive, Bldg. 27, Attn: Ms. Sandra Bennett, Hampton, VA 23667. Telephone responses will not be accepted. Written responses must be received no later than 1:00 PM (EST), May 24, 2012. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued, it will be announced separately from the responses to this announcement.
