Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this effort is VA786-12-R-0149. This requirement is issued as a Request for Proposal (RFP). The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-54. This solicitation is set-aside for small businesses. The North American Industry Classification System (NACIS) Code is 561710, Exterminating and Pest Control Services and the business size standard is $7 million. The period of performance will be for a base period and four option years, if exercised. The Department of Veterans Affairs, National Cemetery Administration intends to award one firm fixed-price contract from this solicitation. In accordance with FAR 15.102-2, a Lowest Price Technically Acceptable source selection process will be conducted. Award will be based on the lowest evaluated price of offerors meeting or exceeding the technical acceptability standard for the non-price factors. If any of the offeror's non-price technical factors are determined to be unacceptable, the proposal shall not be further evaluated. Offerors are advised that the follow Federal Acquisition Regulation (FAR) Clauses are applicable to this procurement: 52.212-1 - Instruction to Offerors - Commercial Items 52.212-2 - Evaluation - Commercial Items 52.212-3 - Offeror Representations and Certifications - Commercial Items 52.212-4 - Contract Terms and Conditions - Commercial Items 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items The following evaluation factors will be used to technically evaluate offers: Factor 1: - Technical a. Project Experience. The offeror shall provide a description of their current or previous experience that is the same or similar in size, scope, and price to this project as outlined in this solicitation. In describing Project Experience, the offeror must provide the following details as it relates to their Project Experience: (1) project title and location; (2) size of project; (3) description of the work performed; (4) reason for any previous delays and/or changes to previous projects; (5) any other pertinent information as it relates to this requirement. b. Key Personnel Experience. The key personnel assigned to this project shall possess the experience and technical competence to perform the requirements of this solicitation. In describing this criteria, at a minimum, the offeror shall provide the following: (1) name of individual; (2) title and position in the company; (3) years of experience as it relates to this requirement; (4) provide detailed description of work experience with project(s) the same or similar in scope, size, and price to this project; (5) position the individual will hold in regards to this project; (6) what percentage of the individual's time will be committed to this project; (7) describe educational background, including degrees, certifications, etc. and provide the name of the granting institution; and (8) any other pertinent information as it relates to this project. Factor 2: State Licensing Requirement The offeror shall provide a copy of the required license(s) required by the State of Illinois to perform the requirement along with their proposal package. Factor 3: Past Performance The offeror shall submit past performance information of three (3) current or previous contracts (within the last five (5) years) for references of the same or similar type projects in scope and size to this solicitation. This information is to be provided on the Past Performance Questionnaire in Section D, Contract Documents, Exhibits, or Attachments of this solicitation. The Government will not research contact phone numbers or emails. If a referral cannot be reached with the information provided, the offeror will not get credit for that referral. However, the absence of past performance information will be evaluated neither favorably nor unfavorably. The attachment associated with this requirement, as listed below, is provided: Attachment A - SF 1449, Solicitation/Contract/Order for Commercial Items Offers are due by May 17, 2012 at 4:00pm Eastern Daylight Time (EDT). Offers shall be submitted by mail or hand-delivery only as specified in Attachment A under Section E. No phone calls will be accepted.
