Description

Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number is VA248-12-Q-1379 and is issued as an Request for Quotes, unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-44. The associated North American Industrial Classification System (NAICS) code for this procurement is 221320 with a small business size standard of $7.00M. This requirement is a [ Small Business ] set-aside and only qualified offerors may submit bids. The solicitation will end on 2012-05-25 12:00:00 Eastern Time. FOB Destination shall be WEST PALM BEACH, FL 33410 The VA VHA West Palm Beach Medical Center requires the following items, Exact Match Only, to the following: This service contract is to provide all labor, tools, materials and special equipment for the service and maintenance of both sewage pump stations located at the Department of Veterans Affairs, West Palm Beach VAMC in accordance with the Statement of Work. Solicitation and Buy Attachments - Statement of Work; Price Schedule Must have a minimum of 5 years experience with operation and maintenance of small and large submersible grinder pumps (like Barnes, Liberty, Homa, and Vaughan) in the range of 2 to 20 horsepower and over all operation of lift station (sewage pumping station) equipment outlined in statement of work. Contractor should be able to identify source of problem for non operation of lift station. Contractor should be able to troubleshoot problem and find a remedy. Contractor should have the resources to install remedy. Offerors must have a minimum of 5 years experience with operation and maintenance or jobs of similar size and scope. Offerors must provide atleast 3 references. All responsible Offerors that respond to this solicitation MUST submit their completed price schedule along with any/all NON-PRICING responses (e.g. technical proposal, representations and certifications, references, etc.) directly to Kari.Jeanlouis-Haliburton@va.gov on or before the closing date and time of this solicitation. Failure to comply with these terms and conditions may result in offer being determined as non-responsive. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition.
