- Performance Work Statement AMENDMENT 1 Description is changed in its entirety to read as follows: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, N40192-12-Q-7082, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-55. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 711510 and the small business size standard is $7.0 M. This is a best value award. The following commercial items are requested in this solicitation: CLIN 0001: Price per service unit ______. Protestant Musician Service in accordance with the attached Performance Work Statement (PWS). Period of Performance (POP): 1 June 2012 - 30 September 2012, billable as Service Units as defined in the PWS, not to exceed 44 Service Units during the POP. CLIN 0001: Price per service unit ______. Protestant Musician Service Rehearsal in accordance with the attached Performance Work Statement (PWS). Period of Performance (POP): 1 June 2012 - 30 September 2012, billable as Service Units as defined in the PWS, not to exceed 44 Service Units during the POP. CLIN 0001 Price per service unit ______. Protestant Musician Special Service in accordance with the attached Performance Work Statement (PWS). Period of Performance (POP): 1 June 2012 - 30 September 2012, billable as Service Units as defined in the PWS, not to exceed 4 Service Units during the POP. CLIN 0001: Price per service unit ______. Protestant Musician Special Service Rehearsal in accordance with the attached Performance Work Statement (PWS). Period of Performance (POP): 1 June 2012 - 30 September 2012, billable as Service Units as defined in the PWS, not to exceed 4 Service Units during the POP. How to Submit Quotes: Offerors are encouraged to submit quotes on any number of the solicited positions from one to all Four positions. Each quoted price must be clearly identified to one and only one of the solicited positions above. To submit a quote: 1. Fill in the price per service unit for each and every position quoted in the space reserved for the price per unit for the respective position in the format provided above, AND 2. Provide supporting document sufficient to demonstrate qualifications (Work History and/or Past Performance Information and/or Resume), AND 3. Submit the price quote information and supporting documents via email to Lorie M. Aguon at Lorraine.Aguon@us.af.mil. Responses to this RFQ must be received via email. Services are to be delivered FOB destination at Andersen AFB Guam 96929. Proposals will be evaluated and awarded to the offeror with the proposal representing the best value to the government based upon experience and price. Experience is more important than price. The provisions at FAR 52.212-1, Instructions to Offerors--Commercial Items is included. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items is also included in this solicitation and the offeror must include a completed copy of this provision with their proposal.
