- 1. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation number RFP12-PHX-03-0001 is being issued as a Request for Proposal (RFP). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-57. The full text of the Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation Supplement (HHSAR) can be accessed via the Internet at http://farsite.hill.af.mil/vffara.htm (FAR) and http://farsite.hill.af.mil/vfhhsara.htm (HHSAR). 4. This solicitation is a total small business set-aside. The North American Industry Classification System (NAICS) code is 337214 - Office Furniture (Except Wood) Manufacturing, and the applicable size standard is 500 employees. 5. This requirement consists of twelve (12) line items: 0001 - Finance, Accounts Receivable, and Contract Health reconfiguration 0002 - Human Resources reconfiguration 0003 - Acquisition Management reconfiguration 0004 - Office of Health Programs reconfiguration 0005 - Office of Environmental Health reconfiguration 0006 - Executive Office reconfiguration 0007 - Time & Materials for Finance, Accounts Receivable, and Contract Health 0008 - Time & Materials for Human Resources 0009 - Time & Materials for Acquisition Management 0010 - Time & Materials for Office of Health Programs 0011 - Time & Materials for Office of Environmental Health 0012 - Time & Materials for Executive Office 6. The Statement of Work and detailed Drawings are attached to this notification. The requirement is for the disassembly, relocation, reassembly, and purchase and installation of modular furniture. **NOTE - THE PRE-BID SITE VISIT IS MANDATORY IN ORDER TO BE ELIGIBLE FOR CONTRACT AWARD.*** The pre-bid site visit will be held at Two Renaissance Square, 40 N. Central Ave., Phoenix, Arizona 85004 on Friday April 13, 2012 at 1:30 p.m. Arizona Standard Time (Arizona does not observe Daylight Savings Time). The fifth floor is a secure floor. All visitors should wait in the lobby nearest the intersection of 1st Avenue and Adams Street. A maximum of 3 attendees per contractor are allowed at the site visit. Notification via E-Mail must be sent to Joshua.Wolery@ihs.gov 24 hours in advance if you wish to attend the site visit. The following information must be transmitted: Name of Contractor, Name of person/s attending, valid E-Mail address and telephone number you can be reached at for notification purposes. 7. The location of contract performance is 40 N. Central Avenue, Phoenix, Arizona 85004, and the point of delivery is FOB Destination to the same address. The Period of Performance is 42 Calendar Days after issuance of Notice to Proceed for the completion of Line Items 0001 through 0006 and 84 Calendar Days after issuance of Notice to Proceed for the completion of Line Items 0007 through 0012. 8. INSTRUCTIONS TO OFFERORS: 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation. Offerors must submit a technical description and proposed schedule of how the work is to be performed, with sufficient detail to allow the contracting officer and technical evaluators to conduct the evaluations and make determinations based on the established evaluation criteria. The description should include how the approach will minimize disrupting operations and impact to Indian Health Service employees. Offerors MUST ALSO SUBMIT A COMPLETED COPY OF THE PRICE SCHEDULE LOCATED AT THE END OF THE SCOPE OF WORK. It must be filled out in its entirety. Line Items 0007 through 0012 will be awarded on a Not to Exceed basis as Time and Materials Line Items. Once the relocation is completed and inventory of parts can be accomplished they will be modified as necessary. 9. PROVISIONS/CLAUSES: 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered: I. Technical a. Approach minimizes disturbances to employees b. Approach adheres to or exceeds schedule requirements II. Past Performance a. Provide three references (include name, telephone, address and email address) III. Price The evaluation factors are listed in descending order of importance.
