- DESCRIPTION: THIS IS NOT A SOLICITATION ANNOUNCEMENT, this is sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide this equipment or equal. This equipment is utilized for the treatment of symptomatic benign prostatic hyperplasia (BPH). Equipment equal to the request must have the following characteristics: (1) Patient may return to normal activities approximately within three (3) days, (2) Length of stay in the hospital be approximately within 24 hours, (3) Length with catheter be approximately within 24 hours. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. Questions should be submitted by email to Daniel.Perez1@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, or large business) relative to NAICS 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers. (Size standard 500 employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on FEDBIZOPPS website. SPECIFIED REQUIREMENT: VA Caribbean Healthcare System, San Juan, Puerto Rico is seeking sources for a potential contractor to provide the following: 01Green Light XPS, Laser System, 0010-0210 to include two laser warning signs and system documentations, 0010-2400. 1 ea 02Glasses, eye protection 532MN PN: 0010-7030 7 ea 03Glasses Laser, Physician 532/1064 PN: 0010-00083 ea 04Video Camera, Insert (1.25 in. Diam) PN: 0010-0721 1 ea 05Video Camera, Insert (.95 in. Diam) PN: 0010-07221 ea 06Annual Gold Service Contract1 ea Responses must be emailed to Daniel.Perez1@va.gov no later than 10:00 am EST, April 20, 2012. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained below. Please place in the subject line of your email "Attention: Laser System". This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the response to this announcement. Include past experience in performing these services/providing the equipment to the VA, other Government (Federal or State) agency, or for a private medical facility.
