- Contractor Badge Affidavit Mow Plan Performance Work Statement - Grounds Maintenance Service Contractor Security Performance Guidline Contractor's Handbook Solicitation W91SMC-12-T-0006 (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. (II) Solicitation W91SMC-12-T-0006 is being issued as a Request for Proposal. (III) This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 05-58 dated 18 Apr 12. (IV) This acquisition is reserved for small business concerns. The Government intends to issue a single award to one offeror. The associated North American Industry Classification System (NAICS) code for this acquisition is 561730, small business size $7 million. (V) Commercial Item Description: The 182 Airlift Wing Illinois Air National Guard is soliciting non-personal services for Grounds Maintenance Services. The contractor is to provide all management, tools, equipment, materials, supplies, transportation, labor, and supervision necessary to accomplish the work in accordance with the Performance Work Statement (PWS) dated 01 Apr 12. The Government intends to award a single Firm Fixed Price contract to the offeror who offers the best value to the Government. Best value considerations will be based on Performance Price Tradeoff (PPT) procedures. At least three past performance references shall be submitted with the following information: project number, date of services, point of contact and telephone number. Basic Contract Period will be on 01 June 2012 through 31 May 2011, with four (4) Option Periods: Option Period 1: 01 Jun 2013 through 31 May 2012; Option Period 2: 01 Jun 2014 through 31 May 2015; Option Period 3: 01 Jun 2015 through 31 May 2016; and Option Period 4: 01 Jun 2016 through 31 May 2017.
