- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation #FA4625-12-Q-0013 is being issued as a Request for Quote. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-56, DPN 20120330, and AFAC 2012-0330. This procurement is 100% Small Business Set-Aside. The applicable North American Industry Classification System (NAICS) Code is 541990. The Small Business Size Standard is $7M. Paper copies of the RFQ will not be made available. Proposals can be e-mailed to the following e-mail address: mailto: eric.corcoran@whiteman.af.mil and paul.denton@whiteman.af.mil. NOTE: Quotations must be received by 10 May 2012, 2:00 PM CST in order to be considered for award; late quotations will not be considered for award. Prospective offerors must be registered in the Central Contractor Registration (CCR) (www.ccr.gov) in order to be eligible for award. All responsible sources may submit a quotation which, if timely received, shall be considered by this agency. Evaluation for this acquistion shall be lowest priced technically acceptable. The contractor shall submit with the price proposal, a technical solution which will be evaluated on a pass fail basis. STATEMENT OF WORK Potable Water Storage Tank Cleaning Bldgs. 2006, 1301, 138, 2003 1. The following Statement of Work is for the Cleaning of the Potable Water Storage Tanks: Bldgs, 2006, 1301, 138, 2003. 2. SCOPE: GENERAL REQUIREMENTS: Contractor shall furnish all labor, tools, material, equipment, and supplies necessary to accomplish the work specified below. Contractor is responsible for keeping construction area(s) neat, orderly, and in compliance with OSHA standards. Contractor shall be responsible for clean up upon project completion. Contractor shall comply with all OSHA, AFOSH, MO10CSR60, and American Water Works Association (AWWA) guidelines. Contractor shall not use Government dumpsters. Contractor shall be responsible for the storage, transportation, and disposal of all demolition and waste materials. The Contractor shall become familiar with all details of the work, verify all dimensions in the field and specifications stated herein and shall advise the Contracting Officer of any discrepancy before performing the work. All work to be accomplished during regularly established Whiteman AFB duty hours. All Contractor personnel are required to have a valid photo identification card to enter the installation. All work shall be coordinated with Base Civil Engineering and Water Treatment Plant personnel. SPECIFICATIONS OF TOWERS 1. Knob Noster Tower (North Tower bld. 138) -= elevated storage tank 208' tall- 62.6' Diameter (3200 square feet) X 35' deep- 750,000 gallons. 2. Windsor Tower (North Tower bld. 138) -= elevated storage tank 208' tall- 62.6' Diameter (3200 square feet) X 35' deep- 250,000 gallons 3. Backwash Tower (water plant compound bld. 2002) -= elevated storage tank 155' tall- 40' Diameter (1500 square feet) X 25.8' deep- 250,000 gallons. 4. Clear Well (water plant compound bld. 2006) -= ground storage tank 64' Diameter (3300 square feet) X 12' deep- 265'000 gallons. PRINCIPLE ITEMS OF WORK: The water tanks shall not be drained as part of the performance of the contract. All work to be accomplished by this project shall comply with the specifications expressed herein and any drawings and/or sketches associated with the project where applicable. All materials to be utilized by the Contractor shall be submitted to the Base Civil Engineer or his official representative for approval IAW the Schedule of Material Submittals. Allow sufficient review time so that installation will not be delayed as a result of the time required to process submittals, including time for re-submittals. The projected inspection/removal period shall be 14 days, starting 4 June 2012. PERMITS/REPORTING: 1. Contractor shall file all required reports in accordance with MO DNR & AWWA where applicable. DISENFECTION of DIVERS: 1. Contractor shall comply fully with AWWA Standard C652-11. INSPECTION/REMOVAL: 1. Removal and disposal of sedimentation/debris in all 4 tanks. 2. Return Tanks and surrounding area to the same condition as before removal/inspection began. 3. Return all keys provided by CEOIA and verify hatches are locked upon completion of work. 4. Test and document CL2 residuals before, during and after inspections. Ensure a Free CL2 residual of at least.5 mg/L exist in tank at conclusion of each tank inspection. Addition of MO DNR approved CL2 Calcium Hypochlorite or Sodium Hypochlorite is the only acceptable method to raise Free CL2 residuals. It shall be contractor responsibility to maintain a minimum CL2 residual of.5 mg/L. If Minimum CL2 residual drops below.5 mg/L the contractor shall provide Bacteriological samples to the State of Missouri for analysis. At no time shall the maximum CL2 residual exceed 4 mg/L. 5. The contractor shall inspect the structural integrity of the tanks to include ladders, shell, roof, vent, man ways, welds, seams, foundation, anchors, safety systems, hatches, external overflow and plumbing according to MODNR, OSHA, EPA, TCEQ and NFPA requirements. 6. The contractor shall create an underwater interior video of all four storage tanks, and it shall be provided to Water Treatment Plant Personnel upon completion of inspection.
