Description
THIS NOTICE IS FOR INFORMATIONAL PURPOSES ONLY. The Centers for Disease Control and Prevention (CDC), National Center on Birth Defects and Developmental Disabilities (NCBDDD), Division of Human Development and Disability contemplates award of a Firm Fixed Price Sole Source Contract to Ground Zero Software, Inc., Palm Springs, CA. The work shall be performed at the Contractor's facility. The Period of Performance shall be for 60 months including a 12 month base period with four 12 month option periods. The North American Industrial Classification Standards (NAICS) Code is 541511 and Standard Business Size of $7M. The sole source award will be conducted in accordance with FAR Part 6.302-1; only one responsible source and no other supplies or services will satisfy agency requirement. This requirement is a continuation of services already provided and is not set aside for small businesses. The purpose of this acquisition is to continue enhancements and maintenance of the Spina Bifida Electronic Medical Record (SB EMR) for use by the CDC as well as the 17 Spina Bifida Clinics funded through two research cooperative agreements: FOA DD11-001 entitled, "National Spina Bifida Patient Registry Clinic Demonstration Project" and FOA DD11-005 entitled, "National Spina Bifida Registry Longitudinal Data Collection and Evaluation." The Electronic Medical Record (EMR) is a medical records solution developed by Ground Zero Software, Inc. customized for 17 Spina Bifida clinics and CDC. The acquisition involves continuation of five major objectives: 1.Enhancements of version upgrades, software patches, expansion of reports and forms, changes to the Public Health Information Network (PHIN) Standards and Surveillance, addition of data fields, ongoing maintenance, user support, user documentation, and training on Ground Zero Software, Inc. proprietary WebTracker software 2.Current software license agreements for Unique ID Generator, Exago Report Writer, and First Databank used in the SB EMR 3.Primary Hosting and Disaster Recovery sites and Entrust IdentityGuard Multi-Factor Authentication Solution for the SB EMR 4.Business Associate Agreements with the 17 Spina Bifida Clinics funded through DD11-001 and DD11-005 using the SB EMR 5.Continuation of the escrow agreement #46266 which protects the coding documentation for the SB EMR Ground Zero's WebTracker software has a proprietary architecture and source code complete with custom built screens and special reports specifically customized as the SB EMR for 17 Spina Bifida Clinics currently using the SB EMR to collect surveillance data that is used by the CDC and is embedded in this application. The Spina Bifida EMR is the only customized application developed and used specifically for the collection of Spina Bifida clinical data and the CDC surveillance data through two cooperative agreements DD11-001 and DD11-005. Due to Ground Zero Software's proprietary architecture and source coding, they are the only vendor uniquely qualified to provide the required enhancements, ongoing support, training and maintenance that will be needed during the total period of performance. CDC is not requesting competitive proposals and no solicitation is available. Firms that believe they can provide the services required are encouraged to identify themselves and give written notification to the Contracting Officer no later than 2:00 PM Eastern Standard Time, by April 24, 2012. Determination not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Respondents will assume all financial costs for preparing a response to this notice. The U.S. Government assumes no financial obligation for the preparation and distribution of prepared responses.
