Description
SOURCES SOUGHT The Department of Veterans Affairs, Tucson, AZ, is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; with special interest in seeking Service Disabled Veteran Owned Small Business (SDVOSB); and (3) their size classification relative to the North American Industry Classification System (NAICS) code (561613) for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work required under the proposed contract. LOCATIONS: The VA Southwest Health Care Network VISN 18 requires armored car service for the Agent Cashier Office and the Veterans Canteen Service located at VA Medical Centers in Arizona, New Mexico and Texas. Description of the requirement: The Contract is to provide services to the Agent Cashier Office to include deposits, pick up, change order request in a secured bag Monday through Friday, except major holidays. The Veterans Canteen services include deposit services, Monday through Friday (unless otherwise noted), except major holidays. Anticipated period of performance is July 1, 2012 through June 30, 2013 with four (4) one year option periods. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., Small/SDVOB/WOSB) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Submit Capability Statements by April 16, 2012 no later than 2:00 PM EDT. Submissions must be made via email to: Breysi.Stephens@va.gov or FAX: (520) 629-1817. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER APRIL 16, 2012. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. (5) No Proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s.) Statement of Work VISN 18 Armored Car Services B.1.1. PURPOSE: The intent of this requirement is to obtain Armored Car Services for the Agent Cashier Office and Veterans Canteen Service for the VA Southwest Health Care Network VISN 18. B.1.2. SCOPE OF WORK: Provide Armored Money Car Transportation Service to pickup deposits and deliver deposits in a timely manner for various facilities of the VA Southwest Health Care Network VISN 18 in accordance with the requirements outlined in this Statement of Work. The minimum number of facilities serviced will be one (1); the maximum number of facilities that can participate will be seven (7). Initial service will be provided to the following facilities: (a). New Mexico VA Health Care System (NMVAHC), 1501 San Pedro Drive SE, Albuquerque, NM 87108 (b). Bob Stump VA Medical Center (BSVAMC), 500 U.S. HWY 89 North, Prescott, AX 86313 (c). Veteran Administration Health Care Center (VAHCC), 5001 North Piedras, EL Paso, Texas 79930 (d). Southern Arizona Veterans Affairs Health Care System (SAVAHCS), 3601 South 6th Avenue, Tucson, AZ 85723 (e). Carl T. Hayden Veterans Affairs Medical Center (CTHVAMC), 650 E. Indian School Rd, Phoenix, AZ 85012 (f). Other facilities in VISN 18 may participate at any time during this contract to include: VA Amarillo Health Care System, in Amarillo, Texas and VA Medical Center in Big Springs, Texas (Price/Cost negotiations for these facilities will be conducted at the time service is deemed required and requested) B.1.3. PERIOD OF PERFORMANCE: Armored Car Services will be required for the following period of performance: Base Year: 1 July 2012 through 30 June 2013 Option Year 1: 1 July 2013 through 30 June 2014 Option Year 2: 1 July 2014 through 30 June 2015 Option Year 3: 1 July 2015 through 30 June 2016 Option Year 4: 1 July 2016 through 30 June 2017 B.1.4. HOURS OF OPERATION: Unless otherwise specified all work will be during regular administrative working hours. The Contractor shall provide Armored Car Services Monday through Friday, 8:00 a.m. to 4:30 p.m., excluding federal holidays. All Federal Holidays observed by the VA will not require service. The following holidays are recognized by the VA: New Year's Day, Martin Luther King's Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day and Christmas Day. It should be noted that if a holiday falls on Saturday, it is observed on Friday. If a holiday falls on Sunday, it is observed on Monday. The Contractor will not be required to perform services on National Holidays. When one of the regularly scheduled delivery days falls on any National Holiday the services shall be performed the following workday; except when a Holiday falls on a Friday, the services shall be performed on the preceding Thursday. B.1.5 TYPE OF CONTRACT: Firm fixed price contract
