- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The South Dakota Army National Guard has a requirement for Soldier Medical & Dental Health Assessment and Service Event. The Medical & Dental assessments are required 14-15July12 in Rapid City, SD. See Solicitation for Service Delivery specifics. The contractor is required to provide their own Mobile Medical & Dental Equipment & Units for conducting exams onsite. The contractor is required to be HIPPA qualified and enter assessment data (CACC Access) into MEDPROs & DENCLASS. The request for proposal and requirement details are listed within W912MM-12-T-0014 solicitation Performance Work Statement and attachments. The Government will evaluate each prospective offer utilizing proposed cost for the service event based on the estimated numbers provided. The contractor must be found acceptable based on the factors listed in Clause 52.212-2 in the Solicitation. The contractor will send in items listed in this clause. No award shall be made unless the Contracting Officer makes confirmation of affirmative determination of responsibility for the potential contractor. Experience or knowledge completing Military Medical & Dental Exams is required. Potential contractor must complete the Central Contractor Registration process prior to award of contract. The applicable NAICS is 621399 for Miscellaneous Health Services. Solicitation response date is 19April2012 at 1PM MT.
