- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented through FAC 2005-56 effective 02 April 2012 and additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will is attached for reference only. This is a Request for Proposal (RFP) W912LQ-12-R-0005 for medical services with an initial period of performance of 30 April 2012 through 31 January 2013. This procurement is issued as a small business set aside under the NAICS 621999 with a small business size standard of $10M. The Virginia Army National Guard will award a single firm fixed priced contract for occupational health services conducted at three facilities once per year. Approximately 355 soldiers will require health services each year. The initial service is tentatively scheduled for 8-18 May 2012. This requirement is for one base period with three option years. Proposals will be evaluated inclusive of option years. The Service Contract Act (SCA) of 1965 is applicable to this procurement. As such, the applicable wage determination for the county in which services will be performed, per event, will be made a part of any resulting award. Wage determinations can be accessed on-line at www.wdol.gov. A separate performance work statement (PWS), Quality Assurance Surveillance Plan and price sheet are attached to this synopsis and are hereby made a part of this solicitation and will be made a part of any resulting award. Also attached are all relevant clauses and provisions. Unit prices shall be inclusive of all applicable costs and fees associated with providing the required service. In accordance with Federal Acquisition Regulation (FAR) Provision 52.212-2, included in this solicitation, award will be made to the low price technically acceptable (LPTA) offer.
