- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) is seeking qualified firms to provide donor panel testing services to meet regulations set forth by the Food & Drug Administration (FDA), American Association of Blood Banking (AABB), College of American Pathologists (CAP), The Joint Commission (TJC), Occupational Safety and Health Administration (OSHA). The resultant contractor shall pick up prospective donor's/patient's blood plasma, or other blood component from the James A. Lovell, Federal Health Care Center (FHCC), North Chicago, IL. The blood, plasma, or other blood component samples will then be tested for various diseases such as HIV and Hepatitis. The resultant contractor shall then review and reconcile records and report the test results back to the FHCC within 24 hours. The Great Lakes Acquisition Center (GLAC) is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to accomplish this service. The contractor is expected to demonstrate successful completion of services of similar size, scope and complexity. The expected start date for the required services is on or around July 2012. The solicitation should be available by May 2012. Interested parties shall provide a brief overview of their firm's capabilities and past experience with projects similar in scope and magnitude to this requirement. Capability statements shall include any past experience of similar content and credentials of key staff members. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: a.DUNS number b.Organization Name c.Organization Address d.Point of Contact (including name, title, addresses, telephone number, fax number and Email address) e.Business size f.Socio-economic status Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 621511. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received by the Great Lakes Acquisition Center (GLAC) no later than: April 17, 2012, 2:00 PM, CST.
