- Bond Forms DOL Davis Bacon General Decision Exhibit 6 Exhibit 5 Exhibit 4 Exhibit 3 Exhibit 2 Exhibit 1 Hoh Drawings Hoh Specifications Solicitation 12-102-SOL-00004 Work consists of constructed improvements to the Hoh Water System, including but not limited to: 1. Installation of approximately 12,850 lf of 8" ∅ PVC-C900 Class 150 water distribution main. 2. Installation of approximately 10,250 lf of 4" ∅ PVC-C900 Class 150 water distribution main. 3. Installation of approximately 265 lf of 12" ∅ PVC-C900 Class 150 water distribution main. 4. Installation of approximately 80 lf of 24" ∅ steel cased horizontal under road boring. 5. Installation of (8) 8" gate valves. 6. Installation of (8) 4" gate valves. 7. Installation of (12) 1" air/vacuum valves. 8. Installation of (4) fire hydrants. 9. Installation of (1) water level control valve with vault. 10. Installation of (1) water service line with meter. 11. Clearing vegetation from both sides of well site access road. 12. Clearing and grubbing around treatment building site, well site, and tank site. 13. Construction of well site and treatment building pad site. 14. Construction of water treatment building, building electrical and controls. 15. Construction of degasifier unit and basin. 16. Extending power and telephone service approx. 260 LF to treatment building. 17. Drilling (2) new community wells. Conducting pump test and water testing. 18. Installation of (2) well pumps, level sensors and appurtenances. 19. Extending power to well pumps from treatment building. 20. Installation of approx. 320 lf of 8-foot chain-link fence with barbed wire and gates. 21. Construction of tank access road and construction of tank pad. 22. Construction of 73,000 gallon concrete water storage tank. 23. Resurfacing of disturbed gravel surfaces. 24. Hydro seeding all disturbed areas. 25. Abandoning two existing wells. 26. * Installation of (8) sampling stations. 27. * Abandoning and disposing of the existing well control panel and shelter. 28. * Abandoning and disposing of the existing degasifier tank, tower, control panel and shelter. * Optional bid items. Award is subject to the availability of funds. This solicitation will be procured as an Invitation for Bid in accordance with FAR Part 14 procedures. This solicitation will be available for download on or about April 3, 2012, with bids due approximately 30 days after solicitation issuance. Project Performance Period: Completion time will be 330 calendar days from the date the Notice to Proceed is issued. Estimated construction cost: Between $1,000,000 and $5,000,000.
