- The proposed contract action is for commercial item, which the Naval Medical Center San Diego intends to solicit this requirement as 100% small business set-aside under the authority of Far 6.203. This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-11-T- 0722 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 813110; Size: 7.0m. INTRODUCTION/ SCOPE Background. The Commanding Officer, Robert E. Bush Naval Medical Center, Twentynine Palms, CA, is responsible for providing the spiritual and religious care of patients and staff. 2. CONTRACTOR REQUIREMENTS. The vendor shall be an independent contractor of Pastoal Care Services. The contractor shall: A. Counsel/visit patients and staff. During visitation, the contractor may deliver the sacraments of the church as needed. The contractor (minister) agrees to perform on behalf of the U.S. Government duties of a Minister for spiritual care of active duty military personnel, families, eligible civilian employees, and other beneficiaries per the terms and conditions of the contract. B. Work on a half-time (.5 FTE) as needed basis. The estimated work schedule will be as follows: (1). Work 16 hours / week (Mon-Fri), as required to meet the needs of the patients and staff. The schedule will be made 1 month in advance, approved by the Executive Officer. After hours calls are not covered in this contract, all calls will be referred to the MCAGCC duty chaplain at 760-830-7200. (2). Provide up to 5 memorial services for personnel/ staff related to the Naval Hospital. C. The contractor shall be paid a flat rate for B-1 and additionally per unit for special requirements. There are no minimum guarantees. The contractor shall not accrue leave as a result this requirement (i.e.: the Government shall not pay for accrued leave). (1). Services are not required on federal holidays. (2). Unexplained absences due to illness or other incapacitation will be allowed up to three days per contract year. The contractor shall advise Naval Hospital of such absences. 3. The contractor shall provide a competent Ministerial substitute who will cover during period of contractor absences (not to exceed 15 days / year). Note that the vendor shall be responsible for actions of substitute Minister and shall make any and all required payments for service to substitute Minister. 4. The contractor shall perform, with the parameters of their ordination and the dictates of their faith group. 5. Staff orientation is required and the independent contract minister will attend orientation within the first month of service and up to one (1) day departmental orientation at the discretion of the Command. Note that this requirement may not apply to vendors who have previously attended orientation. 6. Place of Performance. Work shall be performed onboard Robert E. Bush Naval Hospital. Meetings between the contractor and the Executive Officer, or his/her representative, shall be performed on site. 7. Communication. The contractor may be required to meet with Directors, Department Heads, supervisors, and other staff personnel as well as patients and their families in the performance of pastoral responsibilities. The contractor will pursue all administrative matters through the Executive Officer. When warranted as approved by the Executive Officer, the MCAGCC RMD Command Chaplain and/or the Navy Medicine West Chaplain may be consulted. 8. Qualifications. The contractor shall be a validly ordained Minister in good standing with their diocese or religious order. The contractor shall maintain their certification for the entire contract period. The Government reserves the right to request proof of qualification at any time during contract period. (a) Have ecclesiastical endorsement, in writing, from their diocese or religious order; this will be presented to the command for erification. (b) It is preferred that the contractor have at least one (1) unit of continuing professional education or its equivalency. (c) The Minister shall demonstrate reliability, good relational skills and the ability to serve on an ecumenical ministry team. 9. The contracted minister will comply with government policies and regulations in regard to identification badges, parking, and professional decorum. 10. Prior to commencement of the contract, the contractor must provide a physical examination certification of the proposed minister, which states the date on which the physical examination was conducted, the name of the doctor who performed the examination, and a statement concerning physical health of the contractor. The certification must contain the following statement: (a) (Name of Contractor) is suffering from no physical disability/medical condition which would restrict or preclude him from providing services as a Protestant Minister, (b) (Name of Contractor) is suffering from no contagious diseases to include but not limited to tuberculosis, hepatitis, and is not positive for HIV anti-bodies, and (c) The contractor shall be able to read, write, speak and understand English fluently. 11. Guidance. The Executive Officer will provide direct guidance for the contracted minister. 12. Contract personnel shall be eligible for u.s employment. No alien shall be allowed to perform under this contract in violation of the Immigration Laws of the United States. 13. Personal Appearance. The contractor shall be neat, clean, wellgroomed, and in appropriate clergy clothing when in patient care and public areas. The contractor shall display the identification badge provided by security at all times while at work. The contractor shall provide all clothing which is deemed personal. 14. Government Regulations. The contractor shall become acquainted with and obey all pertinent Command, Navy, and DOD regulations, shall perform in a manner which precludes any waste of utilities and shall not use government equipment for personal business. 15. All motor vehicles operated on this installation by the contractor shall be registered with base security and operated according to applicable directives. 16. Absences. The contractor shall advise the Executive Officer, or his acting substitute, of any planned or unplanned absences. The contractor shall provide a competent substitute with equivalent credentials who will provide Ministerial coverage during periods of contractor absences. 17. The contractor, although in fact not a Government employee, shall comply with Executive Order 11222 of May 8, 1965, "Prescribing Standards of Ethical Conduct for Government Officers and Employees," and shall also comply with Department of Defense (DOD) and Department of Navy (DON) regulations implementing this Executive Order. In addition, the contractor shall adhere to all DOD, Navy, and Command policies and regulations during the performance of their duties. Contractor shall not accept tips and / or donations for services rendered. In the event that donations are offered all such donations shall be referred to the Collection Agent. 18. As an independent contractor, all medical, dental and professional Liability insurance is the responsibility of the contractor.
