Meal Plan Statement of Work Combined Synopsis Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. JTFNMEALMAR001 Solicitation document and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-56. 100% small busines set aside. NAICS Code: 721110 Standard size: $7.0 million 52.212-1: Instruction to Offerors Commercial Items - applies to this acquisition. The following addenda is provided to this provision: Request for Quotation should be submitted and shall contain the following information: Solicitation Number; Time Specified for receipt of offers; Name; Address; Telephone Number of Offeror; Terms of the expressed warranty; Price, Payment terms, Any discount terms, F.O.B. destination, CAGE number, DUNS number, Tax ID number and acknowledgement of all solicitation amendments. Quotes must indicate quantity, unit price and total amount. Offerors that fail to furnish the required information, or reject the terms and conditions of the solicitation may be excluded from consideration. All offers are due no later than: 3pm MST, 16 April 2012. Statement of Work for Catered Meals Line Item 1: Vendor shall cater up to 2265 breakfast meals, for up to 45 personnel at each serving (see daily feed plan attachment), at a designated location in Nogales, AZ from 25 April 12 to 16 June 12. Breakfast meals shall be served daily, Monday thru Sunday, starting at 0630 and ending at 0800. Meals may be served buffet and/or full service style. Breakfast meals shall offer a variety of items that shall include fruits, juice, cold cereal, eggs, bacon/sausage/ham, toast, pancakes/waffles and drinks such as milk, coffee and water. Vendor shall be responsible for providing a place for personnel to eat, plates, eating utensils, drinking glasses/cups, napkins and associated condiments for all meals. Cost of catered meals shall include all costs associated with transporting, preparation, and serving the meals, and gratuity. The vendor shall provide a 7-day menu, at a minimum, each week, one week prior of the scheduled contracted meals, to senior representative and/or supervising staff for review and approval. Requirements/Evaluations Factors for Catered Meals: a. Vendor shall provide breakfast meals 7 days a week. b. Vendor shall provide breakfast meals at government provided location. c. Vendor shall provide prepared meals in a certified kitchen and shall possess the appropriate state, county and city licenses and permits to serve food. d. Vendor shall provide past performance references from previous government contracts. e. Vendor must have a minimum of 5 years of experience in the food catering business. f. Vendor shall provide a 7 day sample menu for breakfast and meals for review prior to award. g. Vendor shall provide documentation to verify number of catered meals served. Line Item 2: Vendor shall cater up to 2265 dinner meals, for up to 45 personnel at each serving (see daily feed plan attachment), at a designated location in Nogales, AZ from 25 April 12 to 16 June 12. Dinner meals shall be catered daily, Monday thru Sunday, starting at 1700 thru 1830. Meals may be served buffet and/or full service style. Dinner meals shall consist of servings that shall include the basic food groups; grains, fruits, vegetables, meats, and dairy product along with a desert and dinner drink. Vendor shall be responsible for providing a location for personnel to eat, plates, eating utensils, drinking glasses/cups, napkins and associated condiments for all meals. Cost of catered meals shall include all costs associated with transporting, preparation and serving the meals, and gratuity. The vendor shall provide a 7-day menu, at a minimum, each week, one week prior of the scheduled contracted meals, to senior representative and/or supervising staff for review and approval.
