Description
SOURCES SOUGHT. The Veterans Administration Pittsburgh Healthcare System (VAPHS), University Drive C, Pittsburgh, PA 15240 is currently seeking potential, qualified sources capable of providing Prefilled (anesthesia) Syringes (including Ephedrine 25 mg in 5 ml 0.9% Sodium Chloride in a 5 ml Syringe; Famotidine 20 mg; Famotidine 20 mg 10 ml 0.9% Sodium Chloride 10 ml ; Hydromorphone 1 mg/ml in 30 ml 0.9% Sodium Chloride 30 ml PCA cartridge; Phenylephrine 800 mcg in 10 ml 0.9% Sodium Chloride in 10 ml Syringe; Phenylephrine 20 mg in 250 ml 0.9% Sodium Chloride 250 ml bag; Rocuronium 50 mg repackaged 5 ml in a 10 ml Syringe; Succinylcholine 200mg repackaged 10ml in a 10ml syringe; Vecuronium 10 mg in 10 ml Sterile Water for injection in a 10 ml Syringe or as may be otherwise determined. ) The vendor shall: be either a licensed manufacturer or registered compounding pharmacy (and documented as such); shall provide products and services directly by the vendor and not a subcontracted vendor; shall comply with standard operating procedures of US Pharmacopeia Chapter 797 for sterile compounding; shall provide data to support the extended expiration dating given to each requested medication due to exceeding the Beyond Use Dating (BUD) per USP Chapter 797; shall use fully trained and qualified employees; shall certify final product(s) are verified by a licensed Pharmacist; shall offer vendor hours of operations at the minimum of Monday through Friday, normal business hours, Eastern Standard Time; products and/or services shall be delivered within 96 hours from the time the order is placed; shall accommodate late or early orders; and shall be capable of on-line ordering or faxing procedures. This is a SOURCES SOUGHT notice only and not a request for proposal. The responses to this announcement will assist in determining whether a socio-economic set-aside is feasible. The VAPHS is required by Public Law 109-461 to give first priority to Service-Disabled Veteran Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) concerns if there should be sufficient firms available to compete under a set-aside. If insufficient veteran-owned or small business response is received to establish the basis for setting this action aside for other than large business participation, then the action will be advertised for full and open competition. Please direct any/all questions to the Contract Specialist named herein, via email only, who will coordinate with the Technical representative for clarification as determined necessary and/or appropriate. The NAICS designation is 325412 - Pharmaceutical Preparation Manufacturing, with a small business standard designation of 750 employees, per the U. S. Small Business Administration reference, http://www.naics.com/maicsfiles/Size_Standards_Table.pdf. It is anticipated that the Base Year of this contract will be June 1, 2012 through May 31, 2013 with anticipated additional four (4) one-year renewal options. Responses to this Sources Sought notice must include: (1) Company Name, (2) Point of Contact, (3) Address, (4) Telephone, (5) E-mail address, and (6) Type of Business such as SDVOSB, VOSB, small business, Hub Zone, 8(a), large business, and (7) a brief summary of technical qualification, etc. Vendors who wish to identify their interest to support the effort described herein must notify the Contract Specialist, Theresa.Pietrowski@va.gov no later than 4:00PM EST, Monday, April 9, 2012.
