- The Department of Veterans Affairs, Veterans Health Administration (VHA), Network Contracting Office (NCO) 5, will solicit proposals under Solicitation No. VA245-12-R-0019 for the following: Rural Case Management Outreach for Homeless Veterans and those at risk of Homelessness Background Information The Department of Veterans Affairs (VA) has a goal of ending homelessness among Veterans by 2015. This goal is supported through the placement of Veterans Health Administration (VHA) homelessness services in rural areas, where there is identified community support for this population. Rural or out-of-state homeless Veterans have difficulty or refuse to lease their state to seek services from VA Medical Center (VAMC) Homeless Programs. Returning Veterans and Veterans struggling due to the economy may not have the knowledge or resources to prevent homelessness. A VA homelessness program based in rural communities is needed to outreach, engage, case manage, refer, and educate homeless Veterans and Veterans at-risk of becoming homeless. Homelessness services are needed in rural areas, where it is difficult for homeless Veterans to travel to medical centers for assessment. Challenges with rural transportation and assessment time may leave Veterans stranded overnight away from services in their communities. This overnight absence may also impact current housing in a community shelter or create other social support issues, which in turn may create additional barriers to ending homelessness. Scope and Task Summary This 12-month contract proposes to continue the establishment of a homelessness services program for rural communities in Pocomoke City, MD; Cambridge, MD; Charlotte Hall, MD; and Stephens City, VA by providing homelessness evaluation and coordination with Community-Based Outpatient Clinics (CBOCs) in these areas. Four Licensed Masters of Social Work (MSWs) and/or Licensed Mental Health Counselors (LMHCs) will act as contracted case managers, working out of the CBOCs or appropriate location within the rural community to enhance Veteran participation and to conduct immediate consultations. When Veterans seek assistance or are referred, the case manager and CBOC staff will begin the process of enrollment and coordinate primary care and mental health assessment, as appropriate. Furthermore, the case manager will work with the Veterans to coordinate appropriate VA services and referrals. In addition to providing case management and identification and referral to needed resources, the case managers will also foster partnerships and collaboration among local health and mental health agencies and resources, establish standard operating procedures for working with the target population, develop service agreements with community resources, and advertise Veteran homelessness prevention services. This acquisition is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB).
