Description
Synopsis: Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, James J. Peter VA Medical Center is currently conducting a market survey for qualified firms capable of providing "Maintenance for Automated Logic Control System". The vendor will be responsible for but not limited to; TWO (2) SCHEDULED PREVENTIVE MAINTENANCE INSPECTION TO BE PERFORMED EACH MONTH. THE CONTACT TO INCLUDE/COVER ALL NORMAL WEAR AND TEAR. INCLUDE ALL LABOR, PARTS MATERIALS NEEDED TO ASSURE THE Automated Logic System IS IN GOOD WORKING ORDER. CONTRACT WILL INCLUDE REPLACEMENT AND LABOR FOR ALL PARTS THAT MALFUNCTION DUE TO NORMAL WEAR AND TEAR. SERVICES REQUIRED TO MAINTAIN SYSTEMS AT MAXIMUM PERFORMANCE AND RELIABILITY LEVELS AND WILL INCLUDE, BUT NOT LIMITED TO, ANALYZE, ADJUST, CALIBRATE THE APPLICABLE SENSORS, HUMIDITY SENSORS, DIAGNOSTIC LEDs, PRINTERS, POWER SUPPLIES, WORK STATIONS, CONTROLLERS, INPUT/OUTPUT POINTS, COMMUNICATION CABLING, TRANSMITTERS, TRANSDUCERS, UPS,FOR THE BMS SYSTEM. All parts and labors. APC recommended service procedure. The North American Industry Classification Systems (NAICS) is 238210. Responses to this notice must be submitting in writing (email or fax) and must be received not later than April 1,st at 10:00AM. No telephone inquiries will be accepted. NOTE: PLEASE PROVIDE CAPABILITY STATEMENT TO THE BELOW EMAIL ADDRESS. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 238210. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). SOW 1 SERVICE CONTRACT: thru 9/30/12 PROVIDE LABOR, MATERIALS AND EQUIPMENT NECESSARY TO PERFORM COMPREHENSIVE MAINTENANCE Automated Logic Control System, Bldg #1 THE CONTRACTOR WILL PROVIDE EMERGENCY SERVICE WITH RESPONSE TIME OF (4) HOURS OF LESS (24/7/365) AT NO ADDITIONAL COST TO THE V.A. OTHER THAN TO COMPENSATE FOR OFF HOURS WORK WHEN APPLICABLE. TO INCLUDE: A. TWO (2) SCHEDULED PREVENTIVE MAINTENANCE INSPECTION TO BE PERFORMED EACH MONTH. SERVICES TO INCLUDE BUT SHALL BE LIMITED TO: SERVICES REQUIRED TO MAINTAIN SYSTEMS AT MAXIMUM PERFORMANCE AND RELIABILITY LEVELS AND WILL INCLUDE, BUT NOT LIMITED TO, ANALYZE, ADJUST, CALIBRATE THE APPLICABLE SENSORS, HUMIDITY SENSORS, DIAGNOSTIC LEDs, PRINTERS, POWER SUPPLIES, WORK STATIONS, CONTROLLERS, INPUT/OUTPUT POINTS, COMMUNICATION CABLING, TRANSMITTERS, TRANSDUCERS, UPS, FOR THE BMS SYSTEM. PERFORM THE ENTIRE PREVENTIVE MAINTENANCE ROUTINE FOR EACH PIECE OF EQUIPMENT AND ALSO, MAKE TESTS AND ADJUSTMENTS TO ENSURE EFFICIENT AND RELIABLE OPERATION OF OTHER MAJOR COMPONENTS. B. Operational Support: Make recommendations to improve system efficiency and reliability or reduce operational costs. A summary of services performed will be prepared for VA review. C. The monthly PM visits will be done during regular working hours: Monday- Friday, 8am-4pm. D. A written report of all PM work done will be provided to the VA. A duplicate record will be maintained by contractor, and will be reviewed with VA personnel on a regularly scheduled basis. The contractor will provide emergency services with a response time of four(4) hours or less (24/7/365)at no additional cost other than to compensate for off-hour work when applicable. E. Update Server WebCTRL software with latest versions and patches as they become available. Notify COTR before updates are applied and accepted. FREQUENCY OF SERVICE: BI-MONTHLY VENDOR PAYMENT SCHEDULE: QUARTERLY VENDOR IS REQUIRED TO OBTAIN VAMC-NY CAMPUS ISSUED IDENTIFICATION BADGES PRIOR TO PERFORMING ANY WORK, EXCEPT IN CASES OF EMERGENCY. VENDOR IS REQUIRED TO CHECK-IN WITH A/C EQUIPMENT OPERATOR FOREMAN OR VENDOR MUST GO TO THE POLICE DESK AND HAVE THEPOLICE OFFICER ON DUTY CONTACT VIA BEEPER THE ENGINEER AT 58-183. VENDOR MUST BE ESCORTED TO WORK SITE BY ENGINEER. VENDOR IS REQUIRED TO SUBMIT OR MAINTENANCE SCHEDULE PRIOR APPROVAL. VENDOR MUST PROVIDE PROOF OF LICENSING AS WELL AS A MINIMUM OF TEN (10) YEARS OF PAST PERFORMANCES SIMILAR TO THIS CONTRACT. ORIGINAL LICENSES MUST BE VIEWED BY COTR AND COPIES MADE FOR FILES. IF THE ABOVE LISTED SERVICE DATES ARE NOT MET, THE VAMC-NY CAMPUS WILL LOOSE THE ABILITY COOL AND/OR HEAT THE MEDICAL CENTER. VENDOR TO SUBMIT (4) COPIES OF REPORTS DETAILING ALL WORK PERFORMED AT V.A. THIS CONTRACT SHOULD BE AWARDED BASED ON SERVICE QUALITY, PERFORMACE AND RELIABILITY. THIS ORDER IS A BONA FIDE NEED OF FY 2012 AND IS ISSUED SUBJECT TO THE AVAILABILITY OF FY 2012 APPROPRIATIONS. All contractor employees and subcontractors under this contract or order are required to complete VA's Information Security and Privacy combined Awareness Training Course annually. Contractors must provide signed certifications of completion to the COTR during each year of the contract.
