Description
Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This is a request for proposal (RFP) No. AG-02NV-S-12-0022; proposal due date is April 10, 2012. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 57, effective as of March 15, 2012. Attached, you will find a Statement of Work for the Computer Support Technician for the Caribou-Targhee National Forest. No later than 3:00 pm MST, April 10, 2012 please provide a price and technical proposal to include proposed personnel resumes, to accomplish the work described in the attached Statement of work. The selection process will consist of a Best Value source selection based on personnel experience, past performance, and price. Under a Best Value source selection, non-price evaluation factors, when combined are significantly more important than price. However, the Contracting Officer will not select an offeror for award on the basis of a superior capability without consideration of the amount of the price. All proposals shall include the following information: 1) Provide resumes of prospective personnel. The personnel will be the person expected to perform the tasks identified in the Scope of Work. The resume shall reflect the personnel's training, certifications and experience with required operating systems as identified in the Scope of Work. Offerors shall submit a Letter of Commitment with each resume for prospective personnel to accept employment on the contract. Failure to submit a signed Letter of Commitment from the perspective personnel with each resume may result in the proposal being considered non-responsive. 2) Provide up to three (3) references for projects that are similar in scope. References may be from commercial and federal/state Government contracts; however, similar support services performed for Federal Government customers generally will be considered more relevant than those done for commercial or state government customers. If a Contractor will be using a subcontractor, up to three (3) references must be provided for the subcontractor as well. The following information is required for each reference: (A) Customer Name and Address (B) Point of Contact (name, telephone number) for contractual/administrative matters & technical performance (C) Period of Contract Performance (D) Description of Work Performed 3) A summary of the Contractor/subcontractor's experience demonstrating the contractor/subcontractor's knowledge and ability to perform the duties and tasks reflected in the Scope of Work. This summary shall not exceed two (2) pages. 4) Describe your organization's policies, procedures, and functional responsibility when selecting and managing employees and subcontractors. 5) If subcontracting, describe how your firm will meet, with its own employees, the Limitations on Subcontracting clause required level of performance of 50%. 6) Price Quote This procurement is a 100% Small Business Set Aside. The NAICS code for this solicitation is 561990. In order to be considered a small business under this NAICS code, your average annual receipts must not exceed $7 Million.
